Study of microRNA significance in colorectal cancer pathogenesis

Abstract:

Kolorektální karcinom (CRC) patří mezi závažná nádorová onemocnění s celosvětově vysokou incidencí i mortalitou. V současné době probíhá řada studií usilujících nejen o identifikaci nových biomarkerů, ale též terapeutických cílů. Jedním z vysoce progresivních přístupů molekulární charakterizace solidních nádorů je analýza mikroRNA (miRNA). MiRNA jsou krátké nekódující RNA regulující expresi až 50 % lidských genů na post transkripční úrovni a jejich deregulace vede ke vzniku mnoha onemocnění, včetně těch nádorových. Předkládaná dizertační práce shrnuje současné poznatky týkající se zapojení miRNA do patogeneze CRC. Pomocí expresního profilování bylo identifikováno šest miRNA, které umožnily spolehlivé odlišení nádorové tkáně od zdravé střevní sliznice. Následně bylo nalezeno šest miRNA, jejichž hladiny byly významně deregulované ve tkáni jaterních metastáz v porovnání s primární nádorovou tkání. Lze proto předpokládat, že tyto miRNA plní důležitou funkci v procesech metastázování. S využitím sekvenování nové generace bylo identifikováno 54 miRNA s rozdílnou expresí v krevním séru pacientů v porovnání se zdravými dárci. Následně byl sestaven diagnostický panel čtyř cirkulujících miRNA, na základě jehož exprese bylo možné odlišit pacienty s CRC od zdravých dárců s vysokou senzitivitou a specificitou. Tento panel zároveň umožnil detekci prekancerózních lézí a jeho citlivost byla významně vyšší v porovnání se současně používanými biomarkery CEA a CA 19-9. V průběhu řešení dizertační práce byl rovněž prokázán prognostický potenciál tkáňových miR-21, miR-215-5p a miR-188-3p a dále byl sestaven prognostický panel cirkulujících miRNA, který by mohl významně přispět k vyšší stratifikaci pacientů na základě jejich prognózy, a usnadnit tak individualizaci léčby. V souvislosti s léčebnou odpovědí pacientů bylo zjištěno, že kombinovaná analýza mutačního stavu onkogenu KRAS a exprese miR 31-5p/3p vede k identifikaci pacientů profitujících z cílené léčby cetuximabem. V závěru práce byla provedena detailní funkční in vitro a in vivo analýza vybraných miRNA, s cílem objasnit jejich zapojení do procesů vzniku a vývoje onemocnění. Získané výsledky prokázaly silný nádorově supresorový charakter miR-215-5p, a lze tedy předpokládat, že tato miRNA by v budoucnu mohla sloužit nejen jako diagnostický a prognostický biomarker, ale též jako slibný terapeutický cíl u pacientů s CRC.

Abstract:

Colorectal cancer (CRC) is a serious disease with high incidence and mortality worldwide. Therefore, several efforts have been made to find new biomarkers as well as therapeutic targets. One of the promising approaches to fulfil these objectives is the characterization of the solid tumors using microRNAs (miRNAs). MiRNAs are small non coding RNAs that regulate gene expression on post-transcriptional level and their deregulation leads to the development and progression of many diseases including cancer. This PhD thesis provides an overview of the current knowledge about the miRNAs’ involvement in CRC pathogenesis and further summarizes author’s contribution to this field. Using expression profiling, we have identified six miRNAs that enabled reliable differentiation between tumour tissue and adjacent non-tumour tissue. Subsequently, six miRNAs with deregulated expression in the tissue of liver metastases compared to primary tumours have been found. Therefore, we concluded that these miRNAs play an important role in the development of metastases. Using next generation sequencing, we have identified 54 miRNAs significantly up-regulated or down-regulated in blood serum of CRC patients compared to healthy donors. Further, the diagnostic panel comprising of four miRNAs has been established. Based on the expression of this panel we were able to differentiate CRC patients from healthy donors with high sensitivity and specificity. Moreover, it was possible to identify the precancerous lesions and the sensitivity of miRNA panel was significantly higher compared to currently used biomarkers CEA and CA 19-9. Also, the prognostic value of miR-21, miR-215-5p, and miR-188-3p tissue levels has been proved in the proposed PhD theses. Additionaly, the prognostic panel of circulating miRNAs has been established. We suggest that this panel could contribute to higher stratification of CRC patients accordingly to their prognosis and consequently increase the level of individualization in the clinical management of CRC patients. Concerning the therapy response, we have demonstrated that combined analysis of KRAS mutation status along with the expression of miR-31-5p/3p leads to the identification of metastatic CRC patients profiting from the targeted therapy using cetuximab. Finally, the detail in vitro and in vivo analysis of selected miRNAs has been performed with the aim to clarify their involvement in the pathogenesis of the disease. The results have proved the strong tumor suppressive properties of miR-215-5p, thus, we suppose that this miRNA could serve not only as a diagnostic and prognostic biomarker, but also as a promising therapeutic target in CRC patients.
