Course of action of Traffic Accidents on the Railway from Emergency Medical Services Perspective

Abstract:

Bakalářská práce se zabývá problematikou polytraumat v přednemocniční neodkladné péči, konkrétně při dopravních nehodách na železnici z pohledu zdravotnické záchranné služby. Je rozdělena na část teoretickou a praktickou. Teoretická část je rozdělena do 3 kapitol, z nichž se každá věnuje jiné části této tematiky. První kapitola definuje dopravní nehody a jim specifické úrazy, hovoří o legislativním rámci dopravních nehod a o činnosti složek integrovaného záchranného systému na místě takové události. Druhá kapitola je zaměřená na železnice a jejich zabezpečení. Železniční dopravní nehody popisuje z pohledu statistického. A poslední, třetí kapitola se věnuje obecně úrazům, specifikuje centra vysoce specializované traumatologické péče, přednemocniční péči o trauma a některé nebezpečné, ale typické úrazy. V praktické části jsou pomocí kvalitativního výzkumu dvou kazuistik převedeny poznatky z teoretické části do praxe. Výzkum popisuje dva pacienty s polytraumaty z jedné železniční nehody, porovnává je a zjišťuje užití doporučených postupů.

Abstract:

This bachelor thesis deals with the subject of severe trauma in prehospital emergency care, more specifically during traffic accidents on the railway from the perspective of emergency medical services. The thesis is divided into two main parts - theoretical and practical. The theoretical part is divided into three chapters and each of them is focused on different topic. First chapter discusses the definition of traffic accident and associated injuries. It also depicts legislative framework of traffic and cooperation of emergency services like fire brigade, police force and paramedics. The second chapter analyzes railway safety using statistics. The last one, third chapter, deals with specific injuries, delineates unique criteria of material and personnel equipment of trauma centers and also describes pathophysiology, diagnosis and treatment of selected serious traumatic conditions. The practical part analyzes two case studies retrospectively. Using this qualitative study, knowledge from the theoretical part is applied into real situation. Research of a railway accident allows us to compare involved patients and establish use of appropriate algorithm on the scene.
