Hygienic disinfection of hands before treatment of peripheral venous entrances

Abstract:

Bakalářská práce se zabývá problematikou hygienické dezinfekce rukou při ošetřování periferních žilních vstupů. Hlavním cílem bakalářské práce je zjistit míru dodržování hygienické dezinfekce rukou před ošetřováním invazivních vstupů podle standardů zdravotnického zařízení. Teoretická část práce se zabývá přiblížením hygienické dezinfekce rukou, následně detailním popisem periferních žilních vstupů a rolí sestry při ošetřování těchto vstupů. V průzkumné části je popsána metodika průzkumu a analýza dat, která byla získána ze 48 skrytých pozorování respondentů. V rámci průzkumného šetření byly zjištěny nedostatky ze stran praktických sester, které dodržují správně pouze 3 stanovená kritéria, což je vyhodnoceno jako nedostatečné. Všeobecné sestry prokázaly, že vzdělání má vliv a je jedním z faktorů pozitivně ovlivňujících dodržování hygienické dezinfekce rukou při ošetřování periferních žilních vstupů. Konkrétně se jedná o kritéria, která sledovala sejmutí náramků, prstenů a vzhled přirozených nehtů, aplikaci dezinfekčního přípravku k hygienické dezinfekci rukou dle směrnice daného zařízení, správné zvolení dezinfekčního přípravku k provedení hygienické dezinfekce rukou a zda byla dodržena technika provedení hygienické dezinfekce rukou. Nejhůře dopadlo kritérium č. 6, zabývající se provedením hygienické dezinfekce rukou v daném časovém rozmezí. U skupiny všeobecných sester došlo k pochybení u 17 respondentů (70,83 %). U praktických sester se nepodařilo toto kritérium splnit také 17 respondentům (70,83 %). Naopak nejlépe dopadlo kritérium č. 9. Toto kritérium sleduje schopnost sester zhodnotit riziko vzniku infekce u periferního žilního vstupu dle škály Maddon. U skupiny všeobecných sester toto kritérium splnilo 16 respondentů (66,66 %). U praktických sester tento výkon zvládlo splnit 21 respondentů (87,50 %). V závěru jsou zjištění shrnuta a je vytvořeno praktické doporučení pro sestry i management.

Abstract:

The bachelor's thesis deals with the issue of hygienic disinfection of hands in the treatment of peripheral venous access. The main goal of the bachelor's thesis is to determine the degree of compliance with hygienic hand disinfection before treating invasive entrances according to the standards of medical facilities. The theoretical part of the work deals with the approach of hygienic disinfection of hands, followed by a detailed description of peripheral venous accesses and the role of the nurse in the treatment of these inputs. The exploratory part describes the survey methodology and data analysis, which was obtained from 48 hidden observations of respondents. The exploratory survey identified shortcomings on the part of practical nurses, who correctly meet only 3 set criteria, which is assessed as insufficient. General nurses have shown that education has an effect and is one of the factors positively influencing the observance of hygienic hand disinfection in the treatment of peripheral venous access. Specifically, these were the criteria that monitored the removal of bracelets, rings and the appearance of natural nails, application of disinfectant for hygienic hand disinfection according to the directive of the facility, correct choice of disinfectant for hygienic hand disinfection and whether the technique of hygienic hand disinfection was followed. Criterion No. 6, dealing with the implementation of hygienic hand disinfection in the given time range, turned out the worst. In the group of general nurses, 17 respondents (70.83%) were mistaken. For practical nurses, 17 respondents (70.83%) also failed to meet this criterion. On the contrary, criterion No. 9 turned out best. This criterion monitors the ability to assess the risk of infection in peripheral venous access according to the Maddon scale. In the group of general nurses, 16 respondents met this criterion (66.66%). For practical nurses, 21 respondents (87.50%) managed to fulfill this performance. In the end, the findings are summarized, and a practical recommendation is made for nurses and management.
