Physiotherapy at degenerative diseases of vertebral column

Abstract:

Souhrn Název: Fyzioterapie u degenerativního onemocnění páteře Cíl: Cílem práce je přispět k řešení problematiky volby optimálních fyzioterapeutických postupů, konceptů a metod u vybraného klienta s degenerativním onemocněním páteře při volbě konzervativního způsobu léčby. Metodika: Pro svou klientku, která trpí onemocněním zvané spodylolistéza, jsem zvolila metodu SMS tahů dle Smíška (2013). V rámci této metody jsou cviky koncipovány tak, že během nich vždy dochází k zapojení celého svalového řetězce, jdoucího napříč celým tělem klientky. Výhodou je, že necvičíme izolovaně jednotlivé svaly HSSP, které chceme vzhledem k diagnóze posílit, ale zapojíme tyto svaly v rámci celého svalového řetězce, jdoucího z horní končetiny přes trup, kde se nacházejí pro nás kýžené svaly, na dolní končetinu klientky. Zvolila jsem sestavu jedenácti cviků, která je vhodná pro vertebropaty. Výsledky: Kineziologický rozbor v rámci výstupního vyšetření prokázal pouze minimální zlepšení a to v těchto parametrech: bilaterálně došlo k uvolnění hypertonického m. trapezius a snížení protrakce ramen (mírné uvolnění m. pectoralis minor) na základě zlepšení dechového stereotypu klientka zapojuje bránici a naučila se používat celou dechovou vlnu, bilaterálně se zvýšila svalová síla do vnitřní rotace v kyčelním kloubu, u klientky došlo také ke zlepšení v provedení testů provokované posturální aktivity (Kolář 2009) - zlepšení nastalo ve dvou ze třech původních patologií, přetrvává pouze insuficience při testu do extenze trupu. Naproti tomu došlo v některých oblastech ke zhoršení stavu: snížení svalové síly v oblasti trupu, kyčelním kloubu (mimo rotace) a kolenním kloubu (flexe), při vyšetření do lateroflexe zůstává stále malý rozsah pohybu a přetrvává pozitivní zkouška Schobera.

Summary: Title: Physiotherapy at degenerative diseases of vertebral column Purpose: My purpose is to redound by troubleshooting with choosing the optimal physiotherapeutic goings, concepts and methods by electing the concervative way of treatment by the chosen client with the degenerative disease of vertebral column. Methodology: My client suffers from the degenerative disease called spondylolisthesis, so this is the reason, why I chozen the method called SMS strokes (Smíšek 2013). Within the framework of this method are exercises drafted in way, that tho whole chain of muscles, which is going across the whole human body, is involved. The advantage is, that we are not doing exercise for every single muscle of the HSSP apart, but we engage these muscles within the framework of the chain of muscles. In this way, empowement of weakened muscles is more effectual. This chain of muscles is going from the upper extremity across the trunk, where the weakened muscles are located, then to the lower extremity. Like a therapy for my client I chose the summary of eleven exercises, which is suitable for people with the disease of vertebral column. Results: The kinesiologic analysis made during check-out proved only minimal improvement in these parameters: bilateral releasing of hypertonic m. trapezius and reduction of protraction position of shoulder joints (slightly release of m. pectoral minor), which were caused by improvement of respiratory stereotype because of the therapy by SMS strokes (Smíšek 2013) - my client engages diaphragma during breathing, the next positive is bilateral increasing of muscle´s strenght of inner rotation in coxal joints and the last positive change is the improvement of practising the special tests of provoked postural activity (Kolář 2009) - my client achieved the improvement in two of three patologicly practised tests. The only test with patological practising stays the exam of trunk extension. On the other hand, there are some regions, where the deterioration of physical condition appears: reduction of muscle´s strenght in regions of the trunk, coxal joints (except for rotations) and knee joints (flexion), then the checkup of lateroflexion, where remains minimal range of movement and at last during providing the exam of Schober remains still positive result, which means bad announcement for my client.
