Possible benefits and contraindications of hippotherapy in children with cerebral palsy

Abstract:

Tato práce je zaměřena na vypracování literární rešerše na téma: Možné benefity a kontraindikace hipoterapie u dětí s DMO. Tato práce obsahuje i průzkum z pohledu rodičů na problematiku indikací hipoterapie u jejich dětí s tímto onemocněním. Dotazovaní klienti jsou účastníky hiporehabilitačních pobytů v Centru Hiporehabilitace Mirákl. Dětská mozková obrna, jinými slovy neprogresivní porucha vyvíjejícího se mozku vede k motorickému handicapu, který ovlivňuje aktivitu dítěte. Označuje skupinu trvalých, neprogresivních poruch vývoje pohybu a držení těla, které způsobují omezení aktivity, k nimž došlo ve vývoji plodu nebo dětském mozku. Motorické poruchy způsobené DMO jsou často doprovázeny poruchami vnímání, cítění a kognice, komunikace a chování, epilepsií a sekundárními problémy pohybového aparátu. Formy dětské mozkové obrny se rozlišují podle charakteru tonusové a následné motorické poruchy. V současnosti neexistuje lék, který by dětskou mozkovou obrnu vyléčil, můžeme ji pouze terapeuticky podpořit a účinně kompenzovat případné vady k ní přidružené. Hipoterapie ve fyzioterapii a ergoterapii je doplňková metoda fyzioterapie, která k léčbě využívá pohyb koňského hřbetu. Vyžaduje neustálé úpravy držení těla klienta. Při pohybu koně dochází k posunu jeho těžiště v sagitální, transverzální a frontální rovině, což způsobuje neustálé kmitání těžiště klienta. Poskytuje posílení podstatných neuroplastických změn v poraněném mozku s významným neurologickým zotavením. Přenos pohybových impulzů v hipoterapii probíhá přes pánev klienta, proto je velmi důležité, aby byla ve správném postavení a v centru pohybu koně. Tyto dva faktory mají zásadní vliv na kvalitu hipoterapeutické jednotky. Dosavadní výzkumy prokázaly, že oborově specifických kontraindikací pro DMO není mnoho, naopak je hipoterapie u dětí s tímto onemocněním velmi intenzivně indikována. Mnohé studie zmíněné v práci doložily, že HTFE přináší klientům s DMO mnoho benefitů. To znamená, že HTFE je ve většině případů doporučenou doplňkovou léčbou, indikovanou klientům s nejrůznějšími typy DMO. Většina kontraindikací spadá do skupiny oborově nespecifických. Oborově specifických kontraindikací je velmi malé množství. Z vlastního dotazníkového šetření uvedených v přílohách vyplynulo, že se dotazovaní rodiče nesetkali u jejich dětí s žádnými kontraindikacemi. Studie uvedené v bakalářské práci dokládají mnohé benefity HTFE a ve studii „Dopad hipoterapie na děti s DMO z perspektivy jejich rodičů“, jsou doloženy přínosy z pohledu rodičů, které jsou v souladu s vlastním setřením autorky, které je přiloženo v přílohách této práce.

Abstract:

This thesis focuses on the development of a literature search on the topic: Possible benefits and contraindications of hippotherapy in children with DMO. This thesis also includes a survey from the perspective of parents on the issue of indications for hippotherapy in their children with this disease. The interviewed clients are participants of hipporehabilitation stays in the Hipporehabilitation Centre Mirákl. Cerebral palsy, in other words, a non-progressive disorder of the developing brain, leads to motor handicaps that affect the child's activity. It refers to a group of permanent, non-progressive developmental disorders of movement and posture that cause activity limitations that occurred in the developing fetal or infant brain. Motor disorders caused by DMO are often accompanied by impairments in perception, sensation and cognition, communication and behaviour, epilepsy and secondary musculoskeletal problems. Forms of cerebral palsy are distinguished according to the nature of the tonus and subsequent motor disorder. At present, there is no cure for cerebral palsy; we can only support it therapeutically and effectively compensate for any associated defects. Hippotherapy in physiotherapy and occupational therapy is a complementary method of physiotherapy that uses the movement of the horse's back for treatment. It requires constant adjustments to the client's posture. As the horse moves, its centre of gravity shifts in the sagittal, transverse and frontal planes, causing the client's centre of gravity to constantly oscillate. It provides enhancement of significant neuroplastic changes in the injured brain with significant neurological recovery. The transmission of motor impulses in hippotherapy is through the client's pelvis, so it is very important that it is in the correct position and in the center of the horse's movement. These two factors have a major influence on the quality of the hippotherapy unit. Previous research has shown that there are not many discipline-specific contraindications for DMO; on the contrary, hippotherapy is strongly indicated in children with this condition. Many of the studies mentioned in this paper have documented that HTFE provides many benefits to clients with DMO. This means that HTFE is in most cases a recommended adjunctive treatment indicated for clients with various types of DMO. Most contraindications fall into the group of non-specific. There are very few field-specific contraindications. From the self-administered questionnaire survey presented in the appendices, the parents interviewed did not encounter any contraindications for their children. The studies cited in the bachelor thesis document the many benefits of HTFE and in the study "The impact of hippotherapy on children with DMO from the perspective of their parents", the benefits from the parents' perspective are documented and are consistent with the author's own findings, which are included in the appendices of this thesis.
