Physiotherapy in patient with Bechterev disease

Abstract:

Cíl práce: Cílem mé práce je přispět k řešení problematiky fyzioterapie u pacienta s onemocněním morbus Bechtěrev při využití vhodných fyzioterapeutických metod, které se dají využít na zmírnění progrese. Metodika: Pro kazuistiku jsem si vybral pacienta se třetím stupněm onemocnění morbus Bechtěrev. Na základě důkladného vstupního kineziologického rozboru (aspekce, antropometrie, goniometrie, statické a dynamické vyšetření páteře, diagnostika pohybového systému, hodnocení dechových funkcí, vyšetření zkrácených struktur, vyšetření chůze), jsem se pokusil sestavit vhodný krátkodobý fyzioterapeutický plán. Fyzioterapeutická intervence proběhla během 10 terapií, při kterých jsem využil dechová cvičení, mobilizační techniky, spinální cvičení, postizometrickou relaxaci (PIR). Při cvičení jsem využil theraband, overball, míčky. Výsledky: Podařilo se znatelně ovlivnit posunlivost fascií, zkrácené a hypertonické svaly. Došlo ke zlepšení mobility hrudníku, aktivaci bránice, zlepšení dechové amplitudy, relaxace i stereotypu dýchání. Veškeré výsledky jsou pak dále zhodnoceny v kapitole 5. Ve výstupním kineziologickém rozboru se potvrdil pozitivní efekt fyzioterapie u pacienta s diagnózou morbus Bechtěrev. Diskuze se věnuje dané problematice. Závěr: V rámci zpracovávání této bakalářské práce, jsem se přesvědčil, že individuální fyzioterapeutická intervence je pro léčbu morbus Bechtěrev prospěšná, stejně jako je potřebný pohyb. Komplexní léčba a pravidelný pohyb zlepšuje kvalitu života pacientů s tímto onemocněním. Cvičením můžeme ovlivnit nově vznikající patologické změny a na pomoc redukovat subjektivní vnímání bolesti. Úspěšnost léčby záleží na spolupráci všech zainteresovaných osob, jako jsou lékaři odborníci, fyzioterapeutové a samotní pacienti.

Abstract:

The aim of the thesis: The aim of my thesis is to contribute to the solution of physiotherapy in patients with Bekhterev s disease using appropriate physiotherapeutic methods that can be used to alleviate its progress. Methodology: For my case study a client with third stage of Bekhterev s disease. Based on a thorough initial kinesiological examination (aspects, anthropometry, goniometry, static and dynamic examination of the spine, diagnostics of the locomotor system, evaluation of respiratory functions, examination of shortened structures, examination of walk). I tried to compile a suitable short-term physiotherapy plan. Physiotherapeutic intervention took place during 10 therapeutical session, in which I used breathing exercises, mobilisation techniques, spinal exercises, and post-isometric relaxation (PIR). During the exercise I used TheraBand, overballs, and balls. Results: I managed to affect fascia, shortened and hypertonic muscles significantly. There also were improvement in thoracic mobility, diaphragm activation, breathing amplitude, relaxation and breathing stereotype. All the results are evaluated in chapter 5. Final kinesiological examination demonstrated the positive effect of physioterapy in a patient with Bekhterev s disease. The discussion deals with the issue. Conclusion: As part of the elaboration of this bachelor's thesis I was convinced that individual physiotherapeutic intervention is beneficial for the treatment of Bekhterev's disease, as well as the need for movement. Comprehensive treatment and regular exercise improve the quality of life of patients with this disease. Through exercise, we can influence emerging pathological changes and help reduce the subjective perception of pain. The success of the treatment depends on the cooperation of all stakeholders, such as specialists, physiotherapists and the patients themselves.
