Cooperation between a home for the elderly and a mobile hospice: Supporting clients and their families in the terminal phase of life

Abstract:

Diplomová práce se zabývá spoluprací domova pro seniory a mobilního hospice, resp. poskytovatele mobilní specializované paliativní péče, při podpoře klientů v terminální fázi života a jejich rodin. Téma vychází z aktuální potřeby empirické evidence, která by detailně popsala zkušenosti pracovníků a rodin a vyústila v doporučení pro sociální práci v prostředí pobytových sociálních služeb a paliativní péče. Teoretická část vymezuje legislativní a institucionální rámec spolupráce sociálního a zdravotního sektoru v České republice, charakterizuje domovy pro seniory a mobilní hospice jako typy služeb s odlišnou organizační logikou a kompetencemi a popisuje konkrétní mechanismy kooperace: komunikační kanály, sdílení dokumentace, vymezení rolí a identifikaci bariér. Součástí teoretického rámce je rovněž problematika podpory klienta a rodiny v terminální fázi - včetně psychosociální, duchovní a praktické dimenze - a role sociální práce v meziinstitucionální koordinaci. Klíčovými dokumenty jsou implementační doporučení GeriPASS pro geriatrickou paliativní péči v pobytových sociálních službách, metodické doporučení Ministerstva zdravotnictví ČR pro organizaci domácí paliativní péče a Strategie rozvoje paliativní péče v ČR do roku 2035. Empirická část má charakter kvalitativní studie. Data byla získána prostřednictvím polostrukturovaných rozhovorů s pracovníky domova pro seniory, pracovníky mobilního hospice a s rodinnými příslušníky klientů, kteří byli přímými účastníky spolupráce obou služeb v terminální fázi. Výběr účastníků byl záměrný s cílem zachytit klíčové profesní perspektivy i perspektivu rodin. Sběr dat proběhl v období od 16. prosince 2025 do 26. ledna 2026. Analýza dat byla provedena tematickou analýzou dle Braun a Clarke. Specifickým aspektem bylo insider postavení autorky jako sociální pracovnice v domově pro seniory, které bylo průběžně reflektováno prostřednictvím analytického deníku a auditní stopy. Výsledky výzkumu identifikují pět vzájemně provázaných témat: proces navázání spolupráce a rozlišení režimu péče, roli komunikačního uzlu a komunikačních kanálů, mechanismy procesního bezpečí, podporu rodin a blízkých a přínosy a bariéry spolupráce. Za klíčový přínos bylo napříč daty označeno posílení procesního bezpečí v zařízení - zejména dostupnost odborné konzultace v režimu 24/7, existence jasného SOS postupu a připravené medikace a dohledatelnost dokumentace podporující jednotné rozhodování ve směnném provozu. Spolupráce současně přispívala k omezení neindikovaných převozů a k podpoře možnosti dožít v prostředí domova. Rodiny vnímaly podporu primárně jako srozumitelnou a dostupnou komunikaci a možnost průběžné orientace v tom, co lze očekávat. Identifikované bariéry se týkají personálních a organizačních podmínek, komunikačního nesouladu při nejasném vymezení rozhraní pro rodinu a potřeby sjednocení postupů prostřednictvím mezioborového vzdělávání. Na základě výsledků jsou formulována doporučení pro praxi sociální práce zaměřená na zavedení jednotného komunikačního rámce pro rodinu při zahájení spolupráce, průběžnou práci s porozuměním cílům péče, posilování přenositelnosti dohod do dokumentace, mezioborové vzdělávání a kazuistickou reflexi a citlivě nabízenou spirituální podporu dle preferencí klienta a rodiny.

Abstract:

This thesis examines the collaboration between a residential care home for older adults and a mobile hospice - that is, a provider of mobile specialised palliative care - in supporting clients in the terminal phase of life and their families. The subject arises from a pressing need for empirical evidence that would describe in detail the experiences of staff and families and translate into recommendations for social work practice within residential social services and palliative care settings. The theoretical section defines the legislative and institutional framework governing collaboration between the social and health sectors in the Czech Republic, characterises care homes for older adults and mobile hospices as service types with distinct organisational logics and competencies, and describes the specific mechanisms of cooperation: communication channels, documentation sharing, role delineation, and the identification of barriers. The theoretical framework also addresses the support of clients and families during the terminal phase - encompassing psychosocial, spiritual, and practical dimensions - and the role of social work in inter-institutional coordination. The principal documents are the GeriPASS implementation recommendations for geriatric palliative care in residential social services, the methodological guidance of the Czech Ministry of Health on the organisation of home palliative care, and the Strategy for the Development of Palliative Care in the Czech Republic to 2035. The empirical section takes the form of a qualitative study. Data were collected through semi-structured interviews with staff from a care home for older adults, staff from a mobile hospice, and family members of clients who had been directly involved in the collaboration between both services during the terminal phase. Participants were selected purposively in order to capture key professional perspectives alongside the perspective of families. Data collection took place between 16 December 2025 and 26 January 2026. Data analysis was carried out using thematic analysis following Braun and Clarke. A specific feature of the study was the author's insider position as a social worker employed in the care home, which was continuously reflected upon through an analytical diary and an audit trail. The findings identify five interrelated themes: the process of establishing collaboration and distinguishing the mode of care; the role of the communication hub and communication channels; mechanisms of procedural safety; support for families and close ones; and the benefits and barriers of collaboration. Across the data, the key benefit identified was the strengthening of procedural safety within the facility - in particular the availability of specialist consultation on a 24/7 basis, the existence of a clear SOS procedure with pre-prepared medication, and the retrievability of documentation supporting consistent decision-making across shift rotations. The collaboration also contributed to a reduction in unindicated hospital transfers and to enabling clients to remain in their home environment until death. Families perceived the support primarily as clear and accessible communication and the opportunity for ongoing orientation regarding what to expect. The barriers identified relate to staffing and organisational conditions, communication misalignment arising from an unclear definition of the interface with the family, and the need to harmonise procedures through interdisciplinary education. On the basis of the findings, recommendations are formulated for social work practice, focused on introducing a unified communication framework for families at the outset of collaboration, ongoing work with understanding the goals of care, strengthening the transferability of agreements into documentation, interdisciplinary education and case-based reflection, and sensitively offered spiritual support in accordance with the preferences of the client and their family.
