Platelet Antibodies in the Gravidity

Abstract:

Diplomová práce nejprve předkládá stručnou charakteristiku trombocytů, trombocytárních antigenů a protilátek. Následně popisuje vyšetřování těchto protilátek a jejich klinický význam. Detailně se zabývá neonatální aloimunní trombocytopenii. V části materiál a metodika je analyzován soubor 24 vzorků (od 23 pacientů) metodu MACE?1 a MACE?2. MACE?1 je kvalitativní ELISA test navržený pro detekci IgG protilátek proti antigenům HLA I.třídy a proti epitopům na glykoproteinech IIb/IIIa destičkových membrán. MACE?2 detekuje IgG protilátky proti epitopům na glykoproteinech Ia/IIa, Ib/IX a IV. V našem souboru vzorků byly u jedné pacientky těmito metodami prokázány IgG protilátky proti epitopům na GPIIb/IIIa, GPIa/IIa a GPIb/IX. U dítěte této matky se potvrdila trombocytopenie s počtem trombocytů po narození 7x109/l. Z dalších závěrů prováděných testů vyplynulo, že pro diagnózu neonatální aloimunní trombocytopenie je dostačující provádět screeningové testy LCT a DIFT. Při pozitivitě v DIFT provést genotypizaci HPA systému a test MACE?1.

Abstract:

This first part of this dissertation presents a brief specification of the platelets, platelet antigens and antibodies. Subsequently, the paper describes the methods of detection of said antibodies and their clinical significance. The neonatal alloimmune thrombocytopenia is analyzed in great detail. A set of 24 samples (collected form 23 patients) is analyzed by the MACE?1 and MACE?2 methods in the Material and Methodology Section. MACE?1 is a qualitative solid phase enzyme linked immunosorbent assay (ELISA) designed to detect IgG antibodies to HLA class I antigens and to epitopes on the platelet glycoprotein IIb/IIIa. MACE?2 is a qualitative solid phase enzyme linked immunosorbent assay (ELISA) designed to detect IgG antibodies to epitopes on the platelet glycoproteins Ia/IIa, Ib/IX and IV. In one particular case from our set of samples, the existence of IgG antibodies against the epitopes on GPIIb/IIIa, GPIa/IIa and GPIb/IX was proven. The diagnosis of this mother?s child confirmed thrombocytopenia with neonatal platelet count 7x109/l. Further outcomes of the test performed lead to the conclusion that the LCT and PIFT screening tests suffice for the diagnosis of neonatal thrombocytopenia. In the case of the PIFT positive test, the HPA genotype and the MACE?1 test are required.
