Comorbidities (sleep and emotional disturbances) of neuropathic pain

Abstract:

Disertační práce na modelu distální symetrické diabetické polyneuropatie (DSPN) představuje neuropatickou bolest (NP) a její komorbidity (deprese, úzkosti a nespa-vost). Komorbidity spolu s NP jsou definovány a jsou představeny možnosti dia-gnostiky a léčby. První cíl: naše observační multicentrická kohortová studie na čtyřech dobře defino-vaných skupinách ukázala signifikantně vyšší prevalence symptomů deprese a úz-kosti u pacientů s bolestivou (46,7 % a 60,7 %) a nebolestivou formou DSPN (24,4 % a 44,4 %) a u diabetiků bez polyneuropatie (22,0 % a 30,0 %) ve srovnání se zdravými kontrolami (7,0 % a 14,1 %). Celkově jsme potvrdili, že NP (a její intenzita) spolu s katastrofizací bolesti jsou dominantní rizikové faktory deprese a úzkosti. Dá-le regresní analýza identifikovala na bolesti nezávislé další rizikové faktory emoční-ho distresu: nižší věk, ženské pohlaví, 2. typ diabetu a závažnost bolestivé DSPN. Druhý cíl: naše intervenční pilotní studie prokázala baterií psychologických dotazní-ků efekt Kognitivně-behaviorální terapie pro nespavost (CBT-I) v redukci nespavosti (32 % až 48 %, bolesti (38 % až 60 %), úzkosti (11 % až 17 %) a deprese (32 %) u pacientů s bolestivou DSPN (model periferní NP), nebo (centrální NP). Polysomno-grafie prokázala u všech pacientů, kteří dokončili kompletní protokol studie, norma-lizaci latence usnutí a u třech i normalizaci spánkové efektivity. Z důvodu neočekáva-telných komplikací pouze 8 pacientů dokončilo kompletní protokol studie, což snížilo statistickou sílu a omezilo interpretovatelnost výsledků. Přesto je výsledný trend konzistentní a koherentní s dostupnými cizími daty.

Abstract:

The thesis uses distal symmetric polyneuropathy (DSPN) in diabetes as a model of neuropathic pain (NP). The text defines comorbidities of NP (depression, anxiety, and insomnia) and introduces the options for their examination and therapy. As the result of our first aim, our observational multi-center cohort study of four well-defined subgroups showed a significantly higher prevalence of the symptoms of depression and anxiety in patients with painful DSPN (pDSPN) (46.7% with de-pression and 60.7% with anxiety), with non-painful DSPN (24.4% and 44.4%), and with diabetes mellitus (DM) without polyneuropathy (22.0% and 30.0%) than in healthy controls (7.0% and 14.1%). In general, we confirmed NP, its severity, and pain catastrophizing as dominant risk factors for depression and anxiety in diabetic patients. Multiple regression analysis identified lower age, female sex, type 2 DM, and pDSPN severity as important contributors to emotional distress independent of pain. As the result of our second aim, our interventional pilot case study employed a battery of questionnaires and polysomnography and showed the effect of cognitive behavioral therapy for insomnia (CBT-I) in reducing insomnia (by 32% to 48%), pain (by 38% to 60%), anxiety (by 11% to 17%), and depression (by 32%) in pa-tients with pDSPN (model of peripheral NP) and multiple sclerosis (central NP). The polysomnography showed the normalization of sleep onset latency in all patients who completed the study and showed the normalization of sleep efficiency in three patients. Due to unexpected circumstances, only eight patients completed the whole study protocol and were analyzed, which decreased the statistical power and limited the interpretation of the results. Despite the small sample size, the trend was con-sistent and coherent with general knowledge.
