Comparison of subtypes of colorectal carcinoma with breast cancer subtypes by correlation of their gene expression profiles.

Abstract:

Genetická heterogenita je jedním z důvodů existence podtypů v rámci jednoho nádorového onemocnění. Následkem této heterogenity se pacienti se stejnou diagnózou mohou lišit jak v odpovědi na terapii, tak v čase přežití. Molekulární podtypy karcinomu prsu byly již dobře charakterizovány a zájmem je zjistit, zda znalosti z této oblasti mohou být použity k vysvětlení heterogenity komplexnějších onemocnění, jako je karcinom kolorekta. Hlavním cílem této práce bylo na základě meta-analytického přístupu nejprve určit, které z genových modulů (skupin genů s korelovanými profily genové exprese), které definují odlišné podtypy karcinomu kolorekta, mohou být zároveň použity ke klasifikaci podtypů karcinomů prsu a následně nově stanovené podtypy (definované těmito moduly) porovnat s klinickými proměnnými a přežitím pacientek. V této práci bylo zjištěno, že 28 z 54 genových modulů definovaných u karcinomu kolorekta, může být použito ke klasifikaci nádorů karcinomu prsu. Tyto moduly souvisí s některými klinickými proměnnými a stanovují pět nových odlišných podtypů karcinomu prsu. Mezi těmito podtypy a většinou klinických proměnných byla zjištěna významná asociace, a také byl pozorován významný rozdíl v době přežití skupin pacientek definovaných těmito novými podtypy. Zjištění tohoto spojení může pomoci porozumět heterogenitě kolorektálního karcinomu a touto cestou by mohl jít další výzkum léčby kolorektálního karcinomu. Například by mohla být použita stejná léčba pro některý z podtypů kolorektálního karcinomu, který je charakteristický stejnými vzory exprese genových modulů jako některý z nových podtypů karcinomu prsu odvozených těmito moduly.

Abstract:

Genetic heterogeneity is one of reasons of the existence of cancer subtypes. Each cancer subtype has its specific features. These depend on some variables, which can have impact on different time of survival or response to therapy, although patients have the same diagnosis. Breast cancer molecular subtypes are well characterized and it is of interest to know, whether this knowledge can be used in explaining heterogeneity of more complex disease, such as colorectal cancer. Main objectives of this work were based on a meta-analytical approach. The first aim was to reveal which of the gene-modules (groups of genes with correlated profiles of gene expression) that determining the differences between the subtypes of colorectal cancer can be used for classification of breast cancer subtypes. Subsequently, to compare subtypes of breast cancer (that are defined by these revealed gene modules) with clinical parameters and patient survival. This work shows that 28 of 54 gene expression modules as defined in colorectal cancer can be reproduced in breast cancer datasets and that they are connected with some clinical features and determine 5 new different subtypes of breast cancer. Significant differences in the association between most of clinical characteristics and new subtypes of breast cancer were observed. In addition, these subtypes were significantly different in survival. These connections can help to understand the heterogeneity of colorectal cancer. The future research of colorectal carcinoma therapy could be led this way. For example the same therapy may be used for some of subtypes of colorectal carcinoma that has the same expression patterns of gene modules as some of new subtypes of breast cancer derived by these modules.
