The influence of work organization of nurses in pressure ulcer prevention in the elderly

Abstract:

Svou bakalářskou práci jsem rozdělila do dvou celků, a to na část teoretickou a část praktickou. Teoretická část se zaobírá problematikou organizace práce a managementu ve zdravotnictví. Dále zde zmiňuji, jaké ve zdravotnictví fungují další různé systémy organizace práce. Jedna kapitola je věnována kompetencím sester. Ve třetí kapitole objasňuji pojem ?senior?, zásady komunikace se seniory a edukaci seniorů. Poslední, ale velmi podstatná kapitola, se zabývá problematikou dekubitů. Hlavním cílem mé bakalářské práce byla identifikace jednotlivých složek a vliv organizace práce na prevenci a ošetřování dekubitů u seniorů. Na základě hlavního cíle jsem si stanovila také tři dílčí cíle. První dílčí cíl měl za úkol zjistit, zda je pro sestry současná organizace práce vyhovující. Druhý cíl zjišťoval, zda mají sestry dostatek pomůcek, vybavení a pomocného personálu pro efektivní vykonávání práce v prevenci a ošetřování dekubitů u seniorů. Poslední dílčí cíl měl za úkol zpracovat na základě odpovědí, návrhů a názorů sester doporučení, jakým způsobem by mohlo dojít ke zlepšení efektivity práce v prevenci a ošetřování dekubitů u seniorů na jejich odděleních. Metodika mé praktické části bakalářské práce byla formou řízeného rozhovoru, ve kterém jsem si připravila otázky, a ty jsem postupně kladla sestrám na odděleních následné péče pomocí řízeného rozhovoru. Sběr dat probíhal od začátku listopadu do konce prosince 2012. Otázky v dotazníku zjišťovaly např. spokojenost sester s rozpisem směn, jaké intervaly polohování užívají ve dne a v noci či zda se sestry účastní seminářů na téma polohování. Pro lepší přehlednost jsem veškerá posbíraná data zpracovala do přehledných tabulek a grafů. Nejdůležitější a nejpodstatnější při zpracování mé praktické části byly informace zjištěné z volných odpovědí, kde měly sestry možnost vyjádřit své nápady, názory či návrhy na zlepšení a efektivitu své práce při prevenci a ošetřování dekubitů u seniorů, na základě kterých bylo vypracováno doporučení pro ošetřovatelskou praxi. Všechny stanovené cíle se podařilo splnit.

Abstract:

I have divided my Bachelor thesis into two parts. The first one is the theoretical one and the second part is the practical one. The theoretical part deals with the issues of organization of work of the managment in the health system. Further I mention here the different systems of organization of work in the health system. One chapter is devoted to powers of nurses. The third chapter clarifies the idea of"senior",the principles of communication with seniors and education of seniors.The last, but very significant chapter, deals with the problem of dekubitus. The main aim of my Bachelor thesis was to identify individual components and the influence of the organization of work at prevention and treatment of decubitus at seniors. From the aim of my main aim I set three main targets.The first one should have found of the conpemporary work for nurses is suitable. The second one was to find if the nurses have enough devices, equipment and auxiliary staff for effective performing work in the treatment of decubitus at seniors. The last aim to compile from the answers, suggestions and opinions of nurses recommendation, which way could lead to the improvement of effecivity of work in the prevention and treatment of decubitus at seniors in their department. Methodology of my practical part of the Bachelor thesis was the form of the controlled dialogue. I prepared the questions and the nurses at the department of after-care were asked. Data collection was under way from the beginning of November to the end of December 2012. The questions in the questionnaire were eliciting, for example, satisfaction of nurses with the duty roster, which intervals the nurses use in positioning during the day and at night or if the nurses take part at the courses about positioning. For better clarity I have worked out all the data into the tabular charts and diagrams. The most important and the most substantial during the working out of the practical part was the information found in the free answers, in which the nurses could express their ideas, opinions or suggestions for improving and effectivity of their work at prevention and treatment decubitus at seniors.According to all the answers, the recommendations for nursing practise were worked out. All the given aims were fulfilled.
