Physiotherapy of shoulder injuries in volleyball players

Abstract:

Úvod: Tématem této bakalářské práce je fyzioterapie u poranění ramenního kloubu u volejbalistů. Tato bakalářská práce je teoreticko-praktická. Při volejbale je vyvolávána vysoká zátěž na ramenní kloub, a to především při opakovaném smečování nebo podávání, a s tím jsou často negativně ovlivněny okolní měkké tkáně. Z tohoto důvodu je poranění ramenního kloubu jedno z nejčastějších poranění ve volejbalovém prostředí, a proto je důležité se této tématice v rámci fyzioterapie věnovat. Cíl: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u volejbalistů s poraněním ramenního kloubu. Metodika: V hlavní části práce je zpracovaná kazuistika 24leté hráčky volejbalu s poraněním ramenního kloubu. Obsahuje vstupní kineziologické vyšetření, v rámci kterého je zahrnuto především odebrání anamnézy, antropometrie, goniometrie, pohybové stereotypy, vyšetření zkrácených struktur, funkční testy. Na základě výsledků byl navržen krátkodobý fyzioterapeutický plán s využitím metod a konceptů fyzioterapie (techniky měkkých tkání, mobilizační techniky, bazální posturální programy, strečink, akrální koaktivační terapie, proprioceptivní neuromuskulární facilitace, instrumentální asistovaná mobilizace měkkých tkání a cvičení s pomůckami). Spolupráce s pacientkou trvala tři měsíce a zahrnovala deset terapií. Terapie probíhaly nepravidelně, každá po dobu 45-60 min. Na konci spolupráce s pacientkou jsem vytvořila výstupní kineziologický rozbor a dlouhodobý fyzioterapeutický plán. Výsledky: Podle subjektivního pocitu pacientky došlo především k úlevě od bolesti. Z výsledků vyplývá, že bylo zlepšeno držení těla pacientky, především držení pravé horní končetiny a lopatky. Došlo ke zvýšení rozsahu pohybu při flexi a zevní rotaci o 5° a abdukce byla větší o 10°, současně se zlepšila svalová síla při vnitřní rotaci, flexi a abdukci pravého ramenního kloubu. Zlepšila se stabilizace lopatek a povedlo se snížit zkrácení svalů horní končetiny. Byly eliminovány trigger pointy a zlepšena posunlivost měkkých tkání v oblasti ramenního pletence. Závěr: Terapii hodnotím poměrně úspěšně. Získala jsem mnoho teoretických poznatků v problematice poranění ramenního kloubu u volejbalistů a praktických poznatků, které jsou účinné pří fyzioterapeutických intervencích.

Abstract:

Introduction: This Bachelor thesis deals with the issue of physiotherapy of shoulder joint trauma in volleyball players. The thesis is theoretical-practical. During volleyball, extreme pressure is exerted on the shoulder joint, especially due to repetitive spiking or serving, which frequently negatively impacts the surrounding soft tissue. That is why the trauma to the shoulder joint is one of the most frequent types of injuries in volleyball and as such it is important to pay attention to this topic within physiotherapy. Aim: The main goal of this Bachelor thesis is to contribute to solving the problem of physiotherapy in volleyball players with shoulder injuries. Methodology: The main part of the thesis presents a case report of a 24-year-old volleyball player with shoulder joint trauma. It includes an initial kinesiological examination, which includes medical history taking, anthropometry, goniometry, movement stereotypes, examination of shortened structures, functional tests, and others. Based on the results, a short-term physiotherapy plan was designed using physiotherapy methods and concepts (soft tissue techniques, mobilisation techniques, basal postural programmes, stretching, acral coactivation therapy, proprioceptive neuromuscular facilitation, instrument-assisted soft tissue mobilisation, and assisted exercise or cupping. The collaboration with the patient lasted three months and included ten sessions. The therapies were irregular, each lasting 45-60 minutes. All therapies took place in the home environment (or within a training unit). A final kinesiological analysis and a long-term physiotherapy plan was created at the end of the collaboration with the patient. Results: According to the patient's subjective sensation, there was relief from the pain and discomfort associated with her injury. The results indicate that the patient's posture was improved, most notably the right upper arm and shoulder blade. There was an increase in flexion range and external rotation by 5° and the abduction was greater by 10°, while muscle strength was also improved with internal and external rotation, flexion and abduction of the right shoulder joint. The stabilisation of the shoulder blades has improved and the shortening of the muscles of the upper limb has been reduced. Trigger points have been eliminated and soft tissue displacement in the shoulder plaiting area has been improved. Conclusion: I rate therapy fairly successfully. I have gained a lot of theoretical knowledge on the issue of shoulder joint injuries in volleyball players and practical knowledge that is effective in physiotherapy interventions.
