The way to home births or else how the birth is becoming a part of everyday life in family

Abstract:

Práce prozkoumává zkušenost žen, které se rozhodly pro plánovaný domácí porod. Zaměřuje se na prožívání matek a proces rozhodování, kterým ženy dospívají k úmyslu porodit doma. Cílem práce je přispět k porozumění fenoménu domácích porodů a přivést k němu výzkumnou pozornost. Teoretická část práce přináší seznámení s dichotomickým pojetím dvou modelů porodní asistence a koncepcí autoritativního vědění. Pozornost je věnována vymezení a praxi domácích porodů a studiím, které se věnují zkušenosti žen rodících doma. Diskutovány jsou studie, které se zabývají domácím porodem z hlediska bezpečnosti a možnosti realizace RCT. Práce obrací pozornost k psychosociálním aspektům porodu, jejichž naplnění zajišťuje bezpečnost, snižuje riziko traumatizace a zvyšuje spokojenost ženy se zážitkem porodu. Diskutována je narativní perspektiva v předávání příběhů porodu mezi generacemi.Výzkum se prostřednictvím pěti rozhovorů s matkami pokouší pomocí interpretativní fenomenologické analýzy rozkrýt, jakými fázemi ženy procházejí než dospějí k rozhodnutí porodit doma. Z analýzy vyplynuly čtyři fáze. Na počátku celého procesu leží trauma nebo zranění, které je z vlastní zkušenosti či transgeneračně přenesené, možností je i alternativní zkušenost zážitku porodu přenesená z předchozích generací. Druhá fáze se vyznačuje vysokou individuální variabilitou témat a zkušeností, jsou zde propojeny procesy reflexe a zpracovávání předchozí zkušenosti, rozvíjení dichotomického myšlení a vnitřního dozrávání k domácímu porodu. Ve třetí fázi rozhodnutí pro domácí porod se objevila u všech žen stejná témata. Jde o podmínky zajištění bezpečnosti a jistoty pro průběh porodu vytvářené souhlasem manžela, důvěrou v porodní asistentku, specifickou oporou v medicíně a oporou v intuici jako autoritativní vědění. Čtvrtou fází je pozitivní zážitek porodu. Ukázalo se, že celý proces je komplexní fenomén. Diskutuji koncept „dost-dobrého“ porodu, významu 4. doby porodní, intuicí jako autoritativním věděním, možností výzkumu sociální reprezentace porodu.

Abstract:

This study is focused on women who decided for a planned homebirth. It concentrates on the experience of mothers and on decision making process that leeds to home delivery. The aim of the study is to understand the phenomenon of planned homebirths and to bring more research attention to it. The theoretical part describes two models of the childbirth assistance and the concept of authoritative knowledge. The attention is paid to the rules and praxis of home birth. Other studies are discused that deal with safety of home birth and with the posibility of doing RCT. The study turns attention to psychosocial aspects of birth, it is discussed that their fullfilment brings more safety, reduces the risk of trauma development and encreases satisfaction of a mother with the experience of birth. The transmission of birth stories from the narrative perspective is discussed. Five women were interwieved about their experience of planned childbirth and how they came to that decision. The data were analyzed by the interpretative phenomenological analysis. There are four phases that emerged from the analysis. First, at the beginning of the process there is a trauma, from own experience or transmitted from mothers to a daughter, there is also a posibility of the transmission of the alternative birth experience. The second phase is characterized by the variability of themes, there are processes of reflection interconnected with processing of traumatic experience, developing dichotomic thinking and inner growing to the decision for a home birth. For the third phase there were the same themes for all women. Those are conditions to ensure security and safety during the delivery, such as husband approval to home birth, trust to a midwife, a specific support from a physician and a hospital, support coming from the intuition as authoritative knowledge. The forth is the positive experience of childbirth. Complexity of the process was showed. The discussion involves the concept of „good enough“ birth, the importace of the period after the birth, intuition as authoritative knowledge, social representation of childbirth.
