The role of midwives in the care of pregnant women dependent on heroin

Abstract:

Těhotenství ženy závislé na heroinu je vždy velice rizikové pro matku i plod, který je ohrožen nejen přímým účinkem drogy, ale také řadou dalších zdravotních a socioekonomických faktorů. Užívání návykových látek má významný vliv na průběh těhotenství, porodu a následný dopad na vývoj dítěte. Takové těhotenství s sebou přináší významné abnormality v péči a role porodní asistentky ve spolupráci se závislou ženou skýtá mnohé odlišnosti. Hlavní zásadou je od počátku navázání kladného vztahu mezi porodní asistentkou a těhotnou ženou založeného na důvěře. Úkolem porodní asistentky není matku přesvědčovat a nutit k abstinenci drog, nýbrž šetrně upozornit na žádoucí odbornou péči, která ženě může pomoci ve snižování dávek drogy, případně v zahájení substituční terapie. Porodní asistentka by měla rozlišovat rizikové skupiny žen, aby se mohla dále odvíjet následná účelná zdravotní a sociální péče, pro které opět platí daná pravidla. Práce obsahuje komplexní analýzu období dvou žen během těhotenství, porodu a období po porodu se zaměřením na identifikaci biopsychosociálních potřeb těchto žen a jejich průběžné uspokojování. Doplněním otázky role porodní asistentky během péče o ženu se závislostí na heroinu jsou zanalyzovány rozhovory se třemi zkušenými porodními asistentkami. Cíly diplomové práce bylo zjistit subjektivní prožívání těhotenství, porodu a období po porodu žen se závislostí na heroinu a zjistit roli porodní asistentky v péči o těhotnou ženu závislou na heroinu a ozřejmit tak náplň práce porodních asistentek.

Abstract:

Pregnant women addicted to heroin is always very risky for the mother and the baby, which is unsafe not only by the effect of the drug, but also many other medical and socio-economic factors. The use of drugs has a main influence on development of pregnancy, giving birth and the impact of the growth of the child. Such pregnancy gives a significant abnormalities which the midwife is about to deal with. The main role of the midwife is to develop a positive relationship with the addicted mother-to-be. A key part is also thoughtfully point out a required professional health care rather than pressuring the mother to stop taking drugs, or alternatively compensating therapy. The midwife should recognize dangerous groups of women for arranging further medical and social treatments. The thesis includes a complex analysis of two women during their pregnancies, giving birth and the period after with the focus on the identification on bio psychosocial needs of these patients and their satisfactions. The adding role of the midwife during the healthcare about the woman addicted to heroin are analysed interviews with three experienced midwifes. The aim of the thesis is to find out subjective experiences of the pregnancy, giving birth and the period after, and discover the role of the midwife during the healthcare for the addicted mother to be, and to understand the role of the midwife in such cases.
