Study of decision making in patients with Parkinson's disease using Iowa Gambling Task (strategy analysis and functional anatomical correlates)

Abstract:

Zabývali jsme se studiem procesu rozhodování u pacientů s Parkinsonovou nemocí za použití testu Iowa Gambling task. Analyzovali jsme strategii v průběhu plnění Iowa Gambling Task u skupiny pacientů s Parkinsonovou nemocí s počátkem onemocnění v mladém věku. Za použití funkční magnetické rezonance (fMRI) jsme následně studovali funkčně anatomické koreláty reakce na výhru i na pokutu. Naše výsledky svědčí pro sníženou schopnost pacientů s Parkinsonovou nemocí s počátkem onemocnění v mladším věku vyvinout efektivní strategii v průběhu plnění testu Iowa Gambling Task, přestože běžné testy u nich neprokazují deficit exekutivních funkcí. Námi navržená alternativní metoda analýzy strategie v průběhu plnění Iowa Gambling Task může být slibnou cestou pro další studium procesů rozhodování. Výsledky fMRI studie svědčí pro alterovanou funkci bazálních ganglií i porušení jejich integrace s kortikálními strukturami zapojenými do limbického okruhu bazálních ganglií u pacientů s Parkinsonovou nemocí v ON stavu v souvislosti s reakcí na pokutu. Naše výsledky poukazují na porušené vnímání pokuty či alterované zpětné hodnocení pokuty u těchto pacientů.

Abstract:

We studied the process of decision making in patients with Parkinson’s disease using Iowa Gambling Task. We analyzed the strategy during the performance of Iowa Gambling Task in a group of patients with early onset of Parkinson's disease. Using functional magnetic resonance imaging (fMRI), we then studied the functional anatomical correlates of response to win on penalties. Our results suggest a decreased ability to develop an effective strategy during the Iowa Gambling Task in the patients with an early onset of Parkinson’s disease, despite the fact that these individuals did not show any executive dysfunction in established neuropsychological tests. The proposed alternative method for the analysis of the strategy adopted during the Iowa Gambling Task may be a promising tool to facilitate further research into decision-making processes. With respect to the responses to a penalty exhibited by the patients with Parkinson’s disease in the ON state, the results of the fMRI study point to an altered function of the basal ganglia and suggest an altered integration with the cortical structures involved in the limbic loop. Such a condition then indicates insufficient perception/negative reinforcement after a loss in these patients.
