The biomechanics of walknig in preschool children

Abstract:

Schopnost bezpečně a efektivně chodit je klíčová pro udržení kvality života. Existuje několik fází, které lze považovat za důležité milníky ve vývoji chůze. Dosud nebylo přesně popsáno, ve kterých fázích krokového cyklu dochází k největším změnám v kinematice chůze. Kvantifikace biomechanických proměnných pomocí Statistical Parameter Maping (SPM) může být vhodným/důležitým nástrojem pro popis rozdílů kinematiky a pro hodnocení pohybových vzorců chůze u dětí. Cílem práce bylo popsat biomechanické rozdíly chůze dětí s normálním vývojem během ontogeneze od 2 do 6 let. Výzkumný soubor průřezových studií byl tvořen 142 dětmi (82 dívek a 60 chlapců) v předškolním věku (studie 1, studie 2). Data byla získána systémem optoelektronické stereofotogrammetrie na 10m dlouhém chodníku, naboso, vlastní preferovanou rychlostí chůze. První studie byla zaměřena na porovnání časoprostorových normalizovaných, nenormalizovaných parametrů a jejich variabilitu. Hodnoty věcné významnosti nenormalizované rychlosti chůze vykazovaly střední velikost vlivu (ES=0,72) při srovnání 2 a 3letých, velkou velikost vlivu při srovnání 2 a 6letých (ES=1,77) a velkou velikost vlivu při srovnání 3 a 6letých (ES=1,22). Parametr normalizované šířky kroku vykazoval statisticky významný rozdíl s velkou velikostí vlivu mezi 2 a 3 roky (ES=1,00), 2 a 6 let (ES=3,17) a 3 a 6 let (ES=1,96). Statisticky významný pokles intraindividuální variability chůze s rostoucím věkem byl pozorován u všech parametrů s výjimkou šířky kroku. Variabilita šířky kroku může u dvouletých dětí sloužit jako parametr pro posouzení odchylek od dětí s běžným vývojem. Druhá studie byla zaměřena na rozdíly v kinematice kloubů dolních končetin během chůze. Na základě SPM byly nalezeny rozdíly v úhlu vnitřní rotace chodidla mezi 2 a 3 a 6letými dětmi. U 2letých dětí byla ve 22-55 % stojné fáze menší rotace chodidla, v 85-100 % stojné fáze byla větší rotace chodidla u 2letých dětí. Dále byla větší vnitřní rotace kolenního kloubu u 2letých v porovnání s 6letým ve 13-25 % stojné fáze a nižší abdukce kolenního kloubu u 2letých v porovnání s 6letým v prvních 13 % stojné fáze. Třetí studie byla zaměřena na ontogenetické rozdíly v kinematice během stojné fáze chůze. Do longitudinální studie bylo zařazeno (n=6) dětí sledovaných po dobu tří let. Analýzou pomocí SPM byly zjištěny významné změny mezi třetím a čtvrtým rokem věku v úhlu chodidla (p<0,001) od 0-96,5 % a v úhlu hlezenního kloubu (p<0,001) ve 100 % stojné fáze. K významným rozdílům v úhlu kolenního kloubu také došlo mezi věkovými skupinami 3 a 5 let (p<0,001) při 100 % a mezi věkovými skupinami 4 a 5 let (p<0,001) od 44 % do 84 % stojné fáze. Tato práce přinesla nové poznatky o biomechanice chůze a jejím vývoji u dětí s normálním vývojem v předškolním věku

Abstract:

The ability to walk safely and efficiently is key to maintaining quality of life. Several stages can be considered important milestones in the development of gait. There is no evidence accurately describing at which stages of the gait cycle the most significant changes in gait kinematics occur. Quantification of biomechanical variables using Statistical Parameter Mapping (SPM) may be a useful tool for describing differences in kinematics and for assessing gait movement patterns in children. This study aimed to describe biomechanical differences in gait in typically developing children during ontogeny from 2 to 6 years of age. The cross-sectional study population included 142 preschool children (82 girls and 60 boys) aged 2-6 years (Study 1, Study 2). Data were acquiied using an optoelectronic stereophotogrammetry system on a 10-m-long sidewalk, barefoot, at the self-selected speed. The first study focused on the comparison of spatiotemporal normalized, and non-normalized parameters and their variability. The values of Effect of size for non-normalized walking speed showed a medium effect size (ES=0,72) when comparing 2- and 3-year-olds, a large effect size when comparing 2- and 6-year-olds (ES=1,77), and a large effect size when comparing 3- and 6-year-olds (ES=1,22). The parameter of normalized stride width showed a statistically significant difference with a large effect size between 2 and 3 years (ES=1,00), 2 and 6 years (ES=3,17), and 3 and 6 years (ES=1,96). A statistically significant decrease in intra-individual variability of gait with increasing age was observed for all parameters except stride width. Step width variability in 2-year-old children can serve as a parameter to assess deviations from typically developing children. The second study focused on differences in lower limb joint kinematics during gait. Based on SPM, differences were found in the foot internal rotation between 2- and 3- and 6-year-old children. In 22-55% of the stance phase, foot rotation was less in the 2-year-old children, and in 85-100% of the stance phase, foot rotation was greater in the 2-year-old children. In addition, there was greater knee joint internal rotation in 2-year-olds compared with 6-year-olds in 13-25% of the stance phase and less knee abduction in 2-year-olds compared with 6-year-olds in the first 13% of the stance phase. The third study focused on ontogenetic differences in kinematics during the stance phase of gait. The longitudinal study included (n=6) children followed for three years. SPM analysis revealed significant changes between the third and fourth year of age in foot angle (p<0.001) from 0-96.5% and in ankle angle (p<0.001) in 100% of the stance phase. Significant differences in knee joint angle also occurred between age groups 3 and 5 years (p<0.001) at 100% and between age groups 4 and 5 years (p<0.001) from 44% to 84% of the stance phase. This study provided new insights into the biomechanics of gait and its development in typically developing prescholl children.
