Management of initial carious lesion

Abstract:

Tato práce se zabývá zejména prvotní fází zubního kazu, kterou v odborné termino-logii nazýváme white spot. Úvod je zaměřen na samotnou definici kazů, jeho katego-rizaci mezi ostatní onemocnění a stručný vhled do problematiky. Pokračuje část o epidemiologické situaci a vývoji tohoto onemocnění v populaci. Zde je práce se zamě-řena zejména na modely, kterými se dá epidemiologie zubního kazu nějak zazname-nat a poté vyhodnotit. Pokračováním je nástin do současné problematiky tohoto onemocnění. Zvláště na narůstající tendenci jeho výskytu. Následuje část o etiologii zubního kazu a stručný popis diagnostiky. Poté je část práce zaměřena na klasifikaci zubního kazu dle anatomického nálezu, vzhled kazů a lokalizaci. Na tuto část plynule navazuje část o procesech demineralizace a remineralizace skloviny a popis dějů s tímto jevem spojených. Je tu i zmínka o fluoridech, které neoddělitelně patří k této problematice. Toto téma je zcela klíčové a odvíjí se od něj pozdější pokračování tera-peutické části práce. V této terapeutické teoretické části je snahou představit nejčas-tější a nejdostupnější možnosti terapie pro běžnou stomatologickou ordinaci dentální hygienistky a zubního lékaře. Jsou zde popsány různé metody léčby v počáteční fázi onemocnění. Zmiňuji zde metody, které jsou zcela neinavzivní jako například ELMEX gel neb přípravky RECALDENT zejména GC MI Paste Plus, GC MI Varnish. Poté jsou zde zmíněny i přípravky které již můžeme zařadit do kategorie mini-invazivních jako jsou ICON, Opalustre, Regenamel – Curadont. V praktické části této práce je u vy-braných, výše popsaných metod zpracovaná kazuistika, která vychází z teoretické části. Hlavním cílem je mít srovnání těchto metod a případného účinku na následnou reparaci. Pacienti pro praktickou část byli vybráni napříč věkovým spektrem, aby-chom ověřili případné použití na co nejvíce věkových skupin pacientů.

Abstract:

This work deals mainly with the initial phase of dental caries, which in technical terminology we call the white spot. The introduction is focused on the very definiti-on of caries, its categorization among other diseases and a brief insight into the is-sue. The section on the epidemiological situation and the development of this disea-se in the population continues. Here I focused mainly on models by which the epi-demiology of dental caries can be somehow recorded and then evaluated. The following is an outline of the current issues of this disease. Especially on the growing tendency of its occurrence. The following is a section on the etiology of dental caries and a brief description of the diagnostics. Then part of the work is focused on the classification of dental caries according to anatomical findings, appearance of caries and location. This part is continuously followed by a section on the processes of de-mineralization and remineralization of enamel and a description of the events asso-ciated with this phenomenon. This topic is absolutely crucial and the later conti-nuation of the therapeutic part of the work follows from it. In this therapeutic theo-retical part, I try to present the most common and most available treatment options for a regular dental office of a dental hygienist and dentist. Various methods of tre-atment in the early stages of the disease are described here. I mention here met-hods that are completely non-invasive, such as ELMEX gel or RECALDENT products, especially GC MI Paste Plus, GC MI Varnish. Then there are also mentioned products that can already be included in the category of mini-invasive ones such as ICON, Opalustre, Regenamel - Curadont. In the practical part of this work, a case study is prepared for selected methods described above, which is based on the theoretical part. The main goal is to have a comparison of these methods and the possible effect on subsequent repair. Patients for the practical part were selected across the age spectrum to verify possible use for as many age groups of patients as possible.
