Abuse of Addictive Substances by the Mentally ill

Abstract:

Cíl: Cílem diplomové práce bylo zjistit zneužívání návykových látek u pacientů s psychiatrickým onemocněním. Dílčí cíle jsou zaměřeny na analýzu rozdílů zneužívání látek u pacientů s jednotlivými vybranými duševními poruchami s ohledem na věk a pohlaví a následně zjistit míru zneužívání alkoholu u vybraných diagnóz a zjistit míru zneužívání nikotinu u vybraných duševních poruch, kteří byli hospitalizováni ve Fakultní nemocnici v Ostravě. Metodika: Pro výzkum byly použity standardizované dotazníky Škála závislosti na alkoholu Alkohol Dependence Scale (ADS) a Fagerströmův test nikotinové závislosti Fagarström Test of Nicotine Dependence (FNTD). Ke zpracování získaných dat byla použita popisná statistika, průměr, směrodatná odchylka, frekvenční tabulky s absolutními a relativními počty. Pro hodnocení průměrného věku dle ADS a FTND byl použit t- test pro dva výběry a analýza rozptylu. Pro hodnocení kategoriálních dat byl použit Fischerův exaktní test. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky: Výzkumným šetřením jsme zjistili, že byl nalezen významný rozdíl ve stupni závislosti na alkoholu podle vybraných klinických diagnóz, zjištěn významný rozdíl ve stupni závislosti na nikotinu podle vybraných diagnóz, nebyl zjištěn významný rozdíl v průměrném věku a pohlaví, nebyla zjištěna významná závislost mezi zneužíváním alkoholu a nikotinu, nebyl zjištěn významný rozdíl v průměrném věku se závislostí na alkoholu a byl zjištěn významný rozdíl v průměrném věku ve stupni závislosti na nikotinu. Závěr: Duševně nemocný může být vystaven takové psychické zátěži, kdy v období dekompenzace duševního stavu bývá náchylný ke zneužívání psychotropních látek, mezi které můžeme řadit alkohol a nikotin. Výzkumné šetření nastiňuje problematiku zneužívání návykových látek, které mají významnou roli v oblasti léčby duševních poruch a spolupráci pacienta na zlepšení duševního stavu.

Abstract:

Objective: The aim of the thesis was to determine the use of addictive substances in patients with a mental illness. Partial objectives of the thesis are aimed and the analysis of differences between abusing substances in patients with individual selected mental disorders, in relation to the age and sex of the patients, and to determine the degree of alcohol and nicotine abuse in selected diagnoses in patients hospitalized at the University Hospital Ostrava. Methods: The following standardized questionnaires were used during the survey: Alcohol Dependence Scale (ADS) and Fagarström Test of Nicotine Dependence (FNTD). The following techniques were used for processing of the obtained data: descriptive statistics, mean, standard deviation, frequency tables with absolute and relative numbers. Furthermore, two-choice t-test and analysis of distribution were used for assessment of the mean age of patients according to ASD and FTND. Fisher?s exact test was used for the assessment of categorical data. The statistical tests were evaluated on the significance level of 5 %. Results: The survey showed a significant difference in the degree of alcohol dependency in relation to selected diagnoses, and a significant difference in the degree of nicotine dependency in relation to selected diagnoses; we did not observe a significant difference in the mean age and sex distribution, nor did we observe a significant dependency between the abuse of alcohol and nicotine, and we also did not observe any significant difference in the mean age of patients with alcohol dependency; however we did observe a significant difference in the mean age of patients with nicotine dependency. Conclusion: A person suffering from a mental illness may be subject to a great deal psychological stress, which may result in the tendency to abuse psychotropic substances during the periods of decompensation, including alcohol and nicotine. The survey outlines the problem of abuse of addictive substances, which plays a significant role in the treatment of mental disorders, and also in the participation of patients upon the improvement of their mental status.
