Cooperation of Emergency Medical Sevices with Complex Cerebrovacular Centres

Abstract:

Bakalářská práce se zabývá součinností zdravotnické záchranné služby s komplexními cerebrovaskulárními centry u pacientů s cévní mozkovou příhodou. Teoretická část se věnuje triáži a léčbě pacientů s cévní mozkovou příhodou v přednemocniční péči, odmítnutí pacienta cílovým zařízením, psychologii a komunikaci záchranáře, která je pro tuto činnost velmi důležitá. Dále pojednává o rozdělení cévních mozkových příhod a možnostech léčby v nemocniční péči. Neméně důležitá je část o vlivu nemoci Covid-19 na cévní mozkovou příhodu. Je zde popsána i strategie péče ve Fakultní nemocnici Motol, Nemocnici Na Homolce a Fakultní nemocnici Královské Vinohrady. Nejdůležitější je praktická část, která obsahuje průzkumné šetření o kooperaci zdravotnické záchranné služby a komplexních cerebrovaskulárních center, z nichž jedno je pavilonového typu. Mapuje, zda se součinností zajistí validita informací o pacientovi, zda se urychlí podání potřebné terapie s následným menším poškozením mozkové tkáně pacienta. Jak vidí zaměstnanci nově zavedenou elektronickou výzvu a důležitost součinnosti v pavilonovém typu nemocnice. Údaje byly zjišťovány od respondentů pomocí anonymního dotazníku, který byl distribuován ve Fakultní nemocnici Královské Vinohrady, Nemocnici Na Homolce a Fakultní nemocnici Motol na neurologických klinikách, které jsou zároveň komplexními cerebrovaskulárními centry. V závěru se snažím o ještě větší osvětu v této oblasti, zvážení změny organizace v pavilonovém systému nemocnice a zkonkretizování elektronické výzvy.

Abstract:

This bachelor´s thesis deals with the cooperation between the Emergency Medical Service and the Complex Cerebrovascular Centers in patients with stroke. The theoretical part focuses on the triage and treatment of patients with stroke in pre-hospital care. It also concentrates on the refusal to accept the patient by the final medical facility, further, on psychology and communication skills of the emergency paramedic, which is a significant part of this job. Next, it discusses the distribution of stroke and treatment options in hospital care. Equally important part of the thesis is dedicated to the influence of Covid-19 disease on the stroke. Another section of the thesis describes health care strategies in Motol University Hospital, Na Homolce Hospital, and Královské Vinohrady University Hospital. The quintessence of the thesis is presented in its practical part which contains a research survey on the cooperation between the Emergency Medical Service and the Complex Cerebrovascular Centers, one of which is a pavilion-style hospital. The survey monitors whether the cooperation ensures the validity of information about the patient and therewith accelerates the administration of necessary therapy resulting in consequent lesser extent of damage to the patient´s brain tissue. Further, the survey reveals how the medical staff perceive a newly introduced electronic alert system – patient´s health information exchange between EMS and a receiving hospital, as well as the importance of the cooperation of the pavilion-style hospital. The data were obtained from the respondents using an anonymous questionnaire which was distributed in Královské Vinohrady University Hospital, Na Homolce Hospital, Motol University Hospital, all in the Departments of Neurology which are also the Complex Cardiovascular Centers. In conclusion, I attempt to raise awareness of the main points of this thesis and to draw attention to potential considering of changes in organization of the hospital pavilion system as well as a precise electronic alert system specification.
