Nursing process in the care of patient with acute renal failure

Abstract:

Bakalářská práce je zaměřena na ošetřovatelský proces u pacienta s akutním selháním ledvin. Cílem bakalářské práce bylo vytvoření plánu péče u konkrétního pacienta tak, aby vyjadřoval jeho individuální potřeby. Sběr anamnézy probíhal pomocí strukturovaného rozhovoru s pacientem dle třinácti domén NANDA-I taxonomie II. Bakalářská práce je složena ze dvou částí - z části teoretické a části praktické. Teoretická část je dále rozdělena na dvě kapitoly. První kapitola se věnuje popisu onemocnění, kde jsou popsány projevy onemocnění, etiologie, léčba a diagnostický postup při vyšetřování pacienta. V podkapitole léčba je část věnována hemodialýze. V druhé teoretické kapitole jsou podkapitoly věnovány specifikaci ošetřovatelské péče na jednotlivých specializovaných pracovištích. Konkrétně na jednotce intenzivní péče, standardní lůžkové stanici a hemodialyzační stanici. Praktická část obsahuje vypracovaný ošetřovatelský proces u pacienta s akutním selháním ledvin během hospitalizace na jednotce intenzivní péče. Třetí kapitola obsahuje formulaci problému, stanovený cíl práce, postup při výběru pacienta a zpracování dat. Mimo jiné zde nalezneme zpracované anamnestické údaje pacienta v jednotlivých třinácti doménách NANDA-I taxonomie II. Na tomto základu bylo stanoveno devět ošetřovatelských diagnóz. U každé z nich byl stanoven cíl péče současně s ošetřovatelskými intervencemi, rozepsanou realizací a hodnocením stanoveného cíle u jednotlivých diagnóz.

Abstract:

This bachelor's thesis is focusing on the nursing practice for a patient diagnosed with acute renal failure. The ultimate goal of this thesis is to create a specific nursing care plan for particular patient to accommodate their individual needs. Details for the nursing care plan were gathered by means of the research through interviews with a patient, according to thirteen domains of the NANDA - I. Taxonomy II. This thesis consists of a theoretical and a practical part. The theoretical part is divided into two chapters. The first chapter looks into the detail and description of the kidney failure and its manifestation, an ethology and a cure with diagnostic procedure for a patient. In the subchapter dedicated to a cure, the majority delved into a hemodialysis. In the practical part of this thesis, the subchapters are focused particularly on nursing care, specifically at an intensive care unit, with a standard bed unit and a hemodialysis unit. The practical part details a specific nursing care plan for a patient with acute kidney failure hospitalised at the intensive care unit. The following chapter consists of problem formulation, setting the goal for the thesis, and the choice of a patient for this research. Patient anamnestic data is enclosed, considering all the thirteen domains of the NANDA I. Taxonomy II. The nine nursing diagnoses were established based on the research. Each nursing diagnosis consist of a nursing care goal, with specific care instructions and practical part with its ranking.
