Abstract:

Úvod Tracheostomie představuje jeden ze základních výkonů u pacientů v intenzivní péči, kteří vyžadují dlouhodobou umělou plicní ventilaci. Ošetřování pacienta s tracheostomií je jedna ze základních dovedností všeobecných sester. Postupy v péči o pacienta s tracheostomií jsou uvedeny ve standardech daného zdravotnického zařízení. Důležitou složkou celkové péče je ošetřování okolí tracheostomie. Zde dochází k odlišným postupům. Dle zahraničních guidelines se doporučuje ošetřovat okolí tracheostomie pouze sterilním fyziologickým roztokem, zatímco v České republice se ve většině zdravotnických zařízení stále preferuje dezinfekce. Metodika V rámci tohoto prospektivního výzkumného šetření autorka realizovala sběr dat z dokumentace pacientů s tracheostomií a následně zaznamenala do výzkumného protokolu. U pacientů s tracheostomií, hospitalizovaných ve Fakultní nemocnici Ostrava na Klinice anesteziologie, resuscitace a intenzivní medicíny, bylo okolí tracheostomie ošetřováno sterilním fyziologickým roztokem. Pacientům s tracheostomií, kteří byli hospitalizováni na Anesteziologicko-resuscitačním oddělení Nemocnice Písek, a. s., se okolí tracheostomie ošetřovalo 2% roztokem chlorhexidinu. Sběr dat u pacientů s tracheostomií zahrnoval vstupní hodnoty, ke kterým patřil věk, pohlaví, délka hospitalizace, hodnota APACHE II, hodnota CRP při přijetí, hodnota CRP v době výskytu lokálních známek zánětu v okolí tracheostomie, hodnoty celkové doby umělé plicní ventilace, doba zavedení a výměny tracheostomické kanyly, způsob odsávání z dýchacích cest, použití tracheostomické kanyly se subglotickým odsáváním, tělesnou teplotu a den, kdy došlo k výskytu lokálních známek zánětu v okolí tracheostomie. Součástí výzkumného protokolu byla zaznamenána i přidružená onemocnění diabetes mellitus a ventilátorová pneumonie. Do studie byli zařazeni pacienti nad 18 let věku, u kterých byla provedena punkční dilatační tracheostomie na primárně sledovaném oddělení v době hospitalizace. Pro statistické zpracování a vyhodnocení byl použit Pearsonův chí kvadrát (?2), test nezávislosti a logistická regrese. Výsledky Na základě prospektivního výzkumného šetření u pacientů s tracheostomií v první experimentální skupině došlo k výskytu lokálních známek zánětu v okolí tracheostomie u 54 % pacientů. Ve druhé, kontrolní skupině, byl zaznamenán výskyt lokálních známek zánětu v okolí tracheostomie pouze u 17 % pacientů. Statistickým zpracováním a analýzou dat byl zjištěn významný rozdíl ve výskytu lokálních známek zánětu v okolí tracheostomie v souvislosti s typem použitého roztoku při ošetřování okolí tracheostomie. Závěr Hlavním cílem této práce bylo zjistit, zda při použití sterilního fyziologického roztoku při ošetřování v okolí tracheostomie dojde k většímu počtu vzniku lokálních známek zánětu než při použití 2% roztoku chlorhexidinu. Hlavní cíl prospektivního výzkumného šetření byl splněn. Autorka zjistila, že u pacientů s tracheostomií, u kterých bylo ošetřováno okolí tracheostomie sterilním fyziologickým roztokem došlo k zvýšenému výskytu lokálních známek zánětu než u pacientů, u kterých byl použit 2% roztok chlorhexidinu.

Abstract:

Introduction Tracheostomy is one of the basic medical procedures performed on patients in a on intensit care unit, who are in need of artificial lung ventilation. Providing care for patiens with a tracheostomy is one of the basic skills for nurses. The standard of the particular medici facility describes how to take care of patiens with a tracheostomy. The treatment of the surrounding tracheal area is an important part of patient care as a whole at this point, however, there are variol practices. According to foreing guidelines it is recommended that the surrounding tracheal areas be treated only with a sterile normal saline while in Czech Republic most medici facilities still prefer disinfection. Methodology The author in this prospective research examination collected data from the documentation of patiens with a tracheostomy, which were noted in the research protocol. Patients with a tracheostomy, who were hospitalized at the Ostrava faculty hospital in the Anesthesiology, Resuscitation and Intensive care Department, the surrounding areas of the tracheal were treated with a sterile normal saline and with patients hospitalized at the Anesthesiological-resuscitation department in the Písek hospital, a. s., the surrounding areas of the tracheal were treated with 2% chlorhexidine solution. Data collection about patients with a tracheostomy included their age, gender, length of hospitalization APACHE II value, CRP value at the time of hospitalization, the time of appearence of local signs of inflammation in the surrounding areas of the tracheostomy, time of duration of the artificial lung ventilation, time of the introduction and exchange of the tracheostomy cannula with subglottic suction and body temperature and day in which the signs of local inflammation in the areas of the performed tracheostomy were noted. As a part of the research protocol, associated diseases such as diabetes mellitus and ventilation pneumonia were also noted. The study contains data of patiens older than 18 year of age, to whom percutaneous dilatational tracheostomy were performed at the highly observed hospital department at the time of thein hospitalization. Pearson chi quadrat test of independence and logistic regression were used for the statistical data processing and evaluation. Result Based on the prospective research examination, 54 % of patiens with a tracheostomy in the experiment group exhibited signs of local inflammation in the tracheal area. In the second control group, only 17 % of patiens exhibited signs of local inflammation in the tracheal area. Through statistical evaluation and data analysis, the differences in the occurrence of local signs of inflammation in the tracheal areas was detected in correlation with the type of the solution used at the time of the treatment of the areas surrounding the trachea. Conclusion The main goal of this study was to discover, whether with the use of sterile normal saline for treatment of the areas surrounding the tracheal higher number of patiens with local signs of inflammation will be detected as in comparison with the use of 2 % chlorhexidine solution.
