The quality of care in patients with infectious bowel diseases

Abstract:

Tématem diplomové práce je Kvalita života pacientů s infekčním onemocněním střev (z anglického jazyka Infectious Bowel Disease, dále i jako IBD). Práce se skládá z teoretické a výzkumné části. V teoretické části je popsána problematika infekčního onemocnění střev, převážně je text zaměřen na Crohnovu chorobu jako nejčastější formu onemocnění střev, její definici, epidemiologii, popis vzniku nemoci, etiologii, diagnostiku, léčbu a také kvalitu života a roli sestry v péči o pacienta. Cílem práce bylo posoudit kvalitu živo-ta vybraného souboru pacientů s IBD v závislosti na sociodemografických charakteristikách pacienta. Dále zjistit sociodemografické faktory, které ovlivňují kvalitu života pacientů s IBD. Data byla získána kvantitativní metodou s využitím standardizovaného dotazníku IBDQ, který je zaměřen na každodenní fungování a kvalitu života respondentů. Pacienti jsou dotazováni na příznaky, které pociťují v důsledku střevního onemocnění. Průzkumem mezi 108 osobami s onemocněním s IBD bylo zjištěno, že mírně pozitivnější hodnocení kvality života v našem souboru uváděli muži. Zajímavé zjištění bylo i to, že respondenti mladší 17 let a starší 81 let dosáhli nižší celkové skóre v hodnocení kvality života. Podle respondentů z našeho souboru má vliv na kvalitu života místo bydliště, při-čemž respondenti žijící na vesnici uváděli lepší kvalitu života než ti, kteří deklarovali, že žijí ve městě. Také pracovní zařazení má u námi dotazovaných respondentů vliv na kvalitu života. Respondenti ve starobním důchodě a pracující na plný úvazek uváděli lepší kvalitu života. Respondenti také uváděli významný dopad na kvalitu života v oblasti emocionálního zdraví a očekávaným zjištěním bylo, že se kvalita života zhoršovala v období relapsu onemocnění.

Abstract:

The topic of the thesis is Quality of life of patients with Infectious Bowel Disease (IBD). The thesis consists of theoretical and research part. The theoretical part describes the problems of infectious bowel disease, mainly the text focuses on Crohn's disease as the most common form of bowel disease, its definition, epidemiology, description of the di-sease, etiology, diagnosis, treatment, quality of life and the role of the setter in patient care. The aim of this study was to assess the quality of life of a selected cohort of IBD patients in relation to patient sociodemographic characteristics The data wwere collected in a quantitative form using a standardized questionnaire, IBDQ, which focuses on the daily functioning and quality of life of the respondents. Patients are asked about the symptoms they experience as a result of their bowel disease. A survey of 108 people with IBD found that slightly more positive quality of life ratings were reported by men in our cohort. Another interesting finding was that respondents younger than 17 years and older than 81 years had lower overall quality of life scores. According to the respondents in our cohort, place of residence has an impact on quality of life, with respondents living in rural areas reporting better quality of life than those who declared living in urban areas. The work assignment also has an impact on the quality of life for our respondents. Re-tired and full-time respondents reported better quality of life. Respondents also reported a significant impact on quality of life in the area of emotional health, and an unexpected finding was that quality of life worsened in the period of disease relapse.
