Clonal diversity of T cells in hematooncological patients

Abstract:

Standardní léčba mnohočetného myelomu (MM) zahrnuje autologní transplantaci hematopoetických kmenových buněk, která umožňuje použít vysoké dávky chemoterapie. Tento léčebný protokol zlepšil klinický výsledek léčby, avšak zároveň u pacientů navyšuje imunosupresi, která byla původně vyvolána samotnou nemocí. S ohledem na zaváděné imunoterapie je třeba zjistit vliv léčby MM na rozmanitost T lymfocytů (TL). Při sledování této rozmanitosti se nejčastěji využívá analýza Vβ rodin T-buněčného receptoru. Cílem naší studie bylo stanovit zastoupení Vβ rodin v buňkách určených k autologní transplantaci a zjistit tak vliv indukční chemoterapie a mobilizace periferní krve na rozmanitost populace TL v organizmu. Pro tento účel byla využita zpětná PCR s novou sadou primerů, která umožňuje detekovat všechny členy Vβ rodin v populaci TL. Relativní exprese Vβ rodin byla následně kvantifikována pomocí kapilární elektroforézy na přístroji Agilent 2100 Bioanalyzer. V okamžiku sběru buněk pro transplantaci bylo u pacientů zjištěno omezené využití Vβ rodin. To bylo definováno na základě srovnání se zdravými dárci a pacienty při diagnóze MM, kteří se naopak vyznačovali rozmanitou expresí Vβ rodin. Zdá se, že indukční chemoterapie a/či mobilizační režim potlačily TL exprimující méně časté Vβ rodiny, které se do doby sběru buněk nestačily obnovit. Ovlivněné TL se nacházejí v transplantátu pro autologní transplantaci a je pravděpodobné, že jejich omezená rozmanitost má negativní vliv na obnovu populace TL po transplantaci. Naše výsledky přispívají k současné snaze pochopit vztah mezi léčbou MM a rozmanitostí TL a zajistit u pacientů co nejrychlejší obnovu populace TL po transplantaci. Získané údaje naznačují, že by pozornost měla být upřena také na přípravný režim pro autologní transplantaci a stav TL v transplantátu.

Abstract:

The current standard of treatment of multiple myeloma (MM) patients includes autologous stem cell transplantation, which enables the use of high doses of chemotherapy. This therapeutic procedure has improved the clinical outcome but simultaneously enhances the immunosuppression initially caused by MM itself. With regard to the current trend of immunotherapy for MM treatment, it is necessary to determine the effects of the therapy on T cells. Vβ gene families are most often analysed when monitoring T-cell population diversity. The aim of our study was to determine expression of Vβ families in cells intended for transplantation and therefore to evaluate the effects of initial chemotherapy and the mobilisation regimen on T-cell population diversity. For this purpose reverse transcription-PCR with a new set of primers enabling specific detection of all Vβ family members was utilized. The relative expression of Vβ families was quantified using capillary electrophoresis performed on a microchip device, the Agilent 2100 Bioanalyzer. In MM patients undergoing peripheral blood stem cell harvest the overall Vβ family usage was found to be restricted. This restriction was defined by comparison with healthy donors and MM patients at diagnosis characterised by diverse Vβ family expression. These results suggested that initial chemotherapy and/or peripheral blood mobilisation had suppressed T cells expressing rare Vβ families and they did not recover by the time of stem cell harvest. Restricted diversity of grafted T cells could negatively affect the recovery of the T-cell population after transplantation. Our work contributes to the current research focused on understanding the relationship between MM treatment regimens and T-cell population diversity and on finding an approach to accelerate the recovery of T-cell population after transplantation. Our results suggested that attention should be among others focused on the preparative regimen for autologous transplantation and the properties of T cells in the graft.
