Diagnosis of lymphedema - comparison between nuclear medicine examination methods and other imaging methods

Abstract:

Teoretická část práce se zabývala porovnáním dnes nejvíce využívané zobrazovací metody v diagnostice lymfatického systému, lymfoscintigrafii, s jinými zobrazovacími metodami. Mezi tyto metody patří například RTG lymfografie, MR lymfangiografie a další. Lymfoscintigrafie je v práci podrobněji rozepsána v samostatné kapitole. Lymfoscintigrafie je dnes nejvíce využívanou zobrazovací metodou při vyšetření lymfatického systému. V praktické části tedy nemělo smysl porovnávat ji s jinými metodami. Hlavním přínosem práce je tedy vynesení předností lymfoscintigrafie nad jinými zobrazovacími metodami. Prokázání, že je opravdu pevným základem v diagnostice lymfatického systému pomocí zobrazovacích metod. Výsledky práce také ukazují, do jaké míry může subjektivní rozhodnutí lékaře o stanovení ROI při semikvantitativním hodnocení lymfoscintigrafie za správně zvolené oblasti zájmu ovlivnit výsledek hodnocení. V práci jsou tedy porovnány i samotné snímky pacientky. Dále bylo zjištěno, zda lze provést hodnocení akumulace radiofarmaka v procentech z AP i PA projekce stejně, nebo bude výsledek rozdílný.

Abstract:

The theoretical part of the work focuses on the comparison of the presently most widely used imaging method in the diagnosis of lymphatic system, lymphoscintigraphy, with other imaging methods. These methods include x-ray lymphography, MR lymphangiography and others. Lymphoscintigraphy is thoroughly discussed in the work in a separate chapter. Lymphoscintigraphy is today the most used imaging method in the examination of the lymphatic system. It was therefore of no importance to compare it with other methods in the practical part of the work. The main contribution of this work is highlighting of lymphoscintigraphy advantages over other imaging methods; proving its rightful place as a solid foundation in the diagnosis of lymphatic system among other imaging methods. The results also show the extent to which subjective judgment of the physician in the determination of ROI in semi-quantitative evaluation of lymphoscintigraphy for correctly chosen area of interest may influence the outcome of the evaluation. The work is thus comparing the actual images of the patient. It was further revealed whether it is possible to equally evaluate radiopharmaceutical accumulation in percentage from AP and PA projections, or the result will be different.
