Quality of Life of Children with Diabetes Mellitus

Abstract:

Cíl: Hlavním cílem výzkumu diplomové práce je zjistit kvalitu života dětí s diabetes mellitus 1. typu s ohledem na věk, pohlaví, délku trvání tohoto onemocnění, a dále zjistit rozdíly v jednotlivých oblastech kvality života mezi dětmi s onemocněním diabetes mellitus 1. typu a dětmi bez tohoto onemocnění. Metodika: Výzkum probíhal formou kvantitativního šetření prostřednictvím obecného standardizovaného dotazníku PedsQL. Dotazník se skládá z 23 otázek rozdělených do 4 oblastí: oblasti fyzických funkcí, psychických funkcí, sociální a školní oblasti. Dotazník byl rozšířen o demografické údaje zaměřující se na věk, pohlaví, délku trvání onemocnění diabetes mellitus 1. typu, pro děti bez chronického onemocnění byl rozšířen pouze o věk a pohlaví. Výzkumný soubor tvořily děti ve věku 8-15 let s diagnózou diabetes mellitus 1. typu, jejich rodiče navštěvující vybrané diabetologické či endokrinologické ambulance a děti bez onemocnění diabetes mellitus či jiného chronického onemocnění navštěvující ordinaci praktického lékaře pro děti a dorost. Výsledky: Z výsledků bylo zjištěno, že onemocnění diabetes mellitus 1. typu významně ovlivňuje kvalitu života dětí v oblasti psychických funkcí, sociální a školní oblasti ve srovnání se zdravou dětskou populací, kde se prokázaly statisticky významné rozdíly. Dále z výsledků vyplývá, že není rozdíl v kvalitě života mezi dívkami a chlapci s diabetes mellitus 1. typu, mezi věkovými kategoriemi 8-12 let a 13-15 let a také, že délka trvání tohoto onemocnění nemá vliv na jejich kvalitu života. Při srovnávání kvality života dětí s diabetes mellitus 1. typu z pohledu jich samotných a z pohledu jejich rodičů se zjistilo, že existuje rozdíl mezi těmito dvěma pohledy, přičemž rodiče hodnotí kvalitu života svých dětí hůře než děti samotné. Závěr: Onemocnění diabetes mellitus 1. typu je velmi závažné chronické onemocnění, které významně negativně ovlivňuje kvalitu života nemocných dětí, ale i pohled na kvalitu života dětí jejich rodiči a vede tak k narušení bio-psycho-sociálních a školních oblastí kvality života. Obecný dotazník PedsQL se prokázal jako jednoduchý, vhodný a spolehlivý nástroj pro měření kvality života zdravé dětské populace, chronicky nemocných dětí a také pro zjištění pohledu rodičů na kvalitu života jejich dětí.

Abstract:

Objective: The main objective of the research is to determine the quality of life of children with type 1 diabetes mellitus with respect to age, sex, duration of this disease, and to find out the differences in various areas of quality of life between children with type 1 diabetes mellitus and children without this disease. Methodology: The research was based on a quantitative survey using a general standardized PedsQL questionnaire. The questionnaire consists of 23 questions divided into 4 domains: physical functions, mental functions, social, and school areas. The questionnaire was complemented with questions on demographic data focusing on age, gender, duration of type 1 diabetes mellitus and in case of children without chronic disease it was complemented with questions on age and gender only. The research file consisted of children aged 8-15 with a diagnosis of type 1 diabetes mellitus, their parents visiting selected diabetology or endocrinology clinics and children without diabetes mellitus or other chronic diseases visiting a general practitioner's office for children and adolescents. Results: The results show that type 1 diabetes mellitus significantly affects the quality of life of children in the mental, social and school areas compared to a healthy paediatric population demonstrating statistically significant differences. Furthermore, the results show that there is no difference in the quality of life between girls and boys with type 1 diabetes mellitus, between the age categories 8-12 years and 13-15 years, nor does the duration of this disease affect the children's quality of life. When comparing the quality of life of children with type 1 diabetes mellitus from their point of view and from the point of view of their parents, it was found that there is a difference between the two perspectives with parents evaluating their children's quality of life worse than the children do themselves. Conclusion: Type 1 diabetes mellitus is a very serious chronic disease which significantly negatively affects the quality of life of sick children, but also the view of the quality of life of children by their parents and thus leads to disruption of bio-psycho-social and school areas of quality of life. The general PedsQL questionnaire proved to be a simple, suitable and reliable tool for measuring the quality of life of a healthy paediatric population, chronically ill children, and also for finding out the parents' view of their children's quality of life.
