Evaluation of Nutritional Status of the Elderly with Chronic Form of Ischemic Heart Disease

Abstract:

Cíl: Cílem práce bylo zhodnotit stav výživy u seniorů s diagnostikovanou chronickou formou ischemické choroby srdeční, hospitalizovaných na kardiovaskulárním oddělení, prostřednictvím použití dvou standardizovaných měřících nástrojů. Metodika: Výzkumné šetření probíhalo formou dotazníkového šetření. Pro získání dat byl použit standardizovaný nástroj Mini Nutritional Assessment (MNA) a Malnutrition Universal Screening Tool (MUST). Dále byly získávány sociodemografické údaje, a údaje o soběstačnosti pomocí Indexu Barthelové (ADL) a Testu instrumentálních všedních činností (IADL). Výzkumný soubor tvořilo 103 respondentů hospitalizovaných na kardiovaskulárním oddělení od dubna do září 2018. Výsledky: Výzkum odhalil výskyt poruch výživy přibližně u 37 % respondentů dle MNA, a přibližně u 26 % dle MUST. Byla zjištěna statisticky významná asymetrie mezi výsledky dle jednotlivých testů (p = 0,028). Celková shoda ve výsledcích byla u 75,7 %, kappa index byl k = 0,44, což lze interpretovat jako "průměrná" statisticky významná shoda. Dále byl zjištěn významný vztah mezi stavem výživy dle MNA a soběstačností dle ADL (p < 0,001), a dle MUST a soběstačností dle ADL (p = 0,001). Významný vztah byl také nalezen mezi stavem výživy dle obou nástrojů a věkem (p = 0,038). Byl zjištěn významný rozdíl mezi časem nutným na vyplnění MNA-SF, MNA a MUST (p < 0,001). Ostatní výsledky nebyly statisticky významné. Závěr: Senioři s chronickou formou ischemické choroby srdeční, hospitalizovaní na kardiovaskulárním oddělení, trpěli poruchami výživy. Poruchy výživy se vyskytovaly více než u třetiny pacientů ze sledovaného souboru. Je tedy nutné věnovat větší pozornost stavu výživy u kardiologických pacientů.

Abstract:

Aim: The aim of this master thesis was to evaluate the nutritional status of elderly patients with diagnosed chronic form of ischemic heart disease, hospitalized at the cardiovascular department, by using two standardized tools. Methods: The research was conducted in the form of a questionnaire survey. The data was retrieved by The Mini Nutritional Assessment (MNA) and Malnutrition Universal Screening Tool (MUST). Furthermore, socio-demographic data and self-sufficiency data were obtained by the Barthel Index (ADL) and the Instrumental Activities of daily living (IADL). The group of respondents consisted of 103 respondents hospitalized at the cardiovascular department from April to September 2018. Results: The research revealed a nutritional disorder in approximately 37% of respondents according to MNA, in 26% according to MUST. Statistically significant asymmetry was determined between results according to these individual tools (p = 0,028). The total value was 75,7%, the kappa index was k = 0.44, which can be interpreted as a 'moderate' statistically significant match. Furthermore, a significant statistical relation was found between nutrition by MNA and self-sufficiency by ADL (p <0,001), and by MUST and self-sufficiency by ADL (p = 0,001). A significant relations was also found between nutrition according to both tools and age (p = 0,038). There was a significant difference between the time for administration of MNA-SF, MNA and MUST (p <0,001). Other results were not statistically significant. Conclusion: The elderly patients with chronic form of ischemic heart disease, hospitalized at the cardiovascular department, suffered from nutritional disorders. Nutritional disorders were found in more than a third of patients in the study group. It is really necessary to pay more attention to the nutritional status of cardiological patients.
