Physiotherapy after total hip arthroplasty

Abstract:

Úvod: Tématem mé bakalářské práce je fyzioterapie po totální endoprotéze (TEP) kyčelního kloubu. Popisuji teoretické poznatky odborné literatury vztahující se k onemocnění a následné léčbě koxartrózy pomocí umělé kloubní náhrady. Má fyzioterapeutická intervence je koncipována na období hospitalizace a po propuštění z hospitalizace. Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie u pacienta po totální endoprotéze kyčelního kloubu. Metodika: V hlavní části kvalifikační práce se věnuji kazuistice 69leté pacientky, která 9. 12. 2022 podstoupila implantaci totální endoprotézy levého kyčelního kloubu. Fyzioterapeutická intervence v období hospitalizace trvala po dobu 7 dní v intervalu 1x denně kromě víkendu a následně po propuštění z hospitalizace v rodinných prostorech určených pro rehabilitaci po dobu 14 dní v intervalu 1x za 3 dny. Délka každé terapie trvala v rozmezí 50-80 minut. V rámci vstupního a výstupního rozboru bylo vyšetřeno držení, konfigurace, trofika, hybnost, kloubní vůle, svalová síla, pohybové stereotypy, chůze a zkrácené struktury pacientky. Ke zlepšení celkového zdravotního stavu pacientky jsem při terapiích použil cévní gymnastiku, míčkovou facilitaci, techniky měkkých tkání, izometrickou aktivaci svalů, pasivní/aktivní pohyb, postizometrickou relaxaci a cvičení proti odporu. Dále bylo využito konceptu Dynamické neuromuskulární stabilizace, proprioceptivní neuromuskulární facilitace, Brügger konceptu, nebo spirální stabilizace a mobilizace páteře. Na konci práce stanovuji dlouhodobý fyzioterapeutický plán. Souhlas se zpracováním osobních údajů byl podepsán pacientkou 12. 12. 2022. Výsledky: V období hospitalizace došlo ke zmenšení otoku levé dolní končetiny, zvýšení hybnosti, svalové síly, zlepšení posunlivosti měkkých tkání v oblasti jizvy a postupnému snižování bolesti. Během pooperační fyzioterapie po propuštění z hospitalizace se nadále zlepšovala celková zdravotní kondice pacientky doprovázena jistotou chůze o dvou francouzských holích (FH). Závěr: Dodržování krátkodobého fyzioterapeutického plánu, vzájemná spolupráce a především autoterapie vedla k navrácení do běžného života, ústupu bolesti a zlepšení psychické pohody pacientky.

Abstract:

Introduction: The topic of my bachelor's work is physiotherapy after total hip arthroplasty. I describe the theoretical pieces of knowledge of scientific literature related to the disease and the subsequent treatment of coxarthrosis using an artificial joint replacement. My physiotherapy intervention is aimed on term of hospitalization and after discharge from hospitalization. The objective: The main goal of the work is to contribute to solving the problems of physiotherapy on a patient after total hip arthroplasty. Methodology: In the main part of the qualification thesis, I focus on the case report of a 69-year-old female patient who underwent implantation of a total arthroplasty of the left hip joint on 9. December 2022. Physiotherapy intervention during the period of hospitalization lasted for 7 days at an interval of 1 a day except the weekend, and subsequently after discharge from hospitalization in family premises intended for rehabilitation for 14 days at an interval of 1 every 3 days. The length of each therapy lasted between 50-80 minutes. As part of the input and output analysis, the patient's posture, configuration, skin condition, dynamics, joint play, muscle strength, movement stereotypes, gait and shortened structures were examined. In order to improve the overall health of the patient, I used vascular gymnastics, soft ball facilitation, soft tissue techniques, isometric muscle activation, passive/active movement, post-isometric relaxation and resistance exercises during the therapies. Furthermore, the concept of Dynamic neuromuscular stabilization, proprioceptive neuromuscular facilitation, Brügger concept, or spiral stabilization and mobilization of the spine was used. At the end of the work I establish a long-term physiotherapy plan. The consent to the processing of personal data was signed by the patient on 12. December 2022. Results: During the hospitalization term, we reached a decrease of the swelling left lower limb, increase in mobility, muscle strength, an improvement in the mobility of soft tissues in the area of the scar, and also reduction in pain. During postoperative physiotherapy after discharge from the hospital, the patient's overall health condition continued to improve, accompanied by the ability to walk with two French crutches. Conclusions: Adherence to a short-term physiotherapy plan, mutual cooperation and autotherapy led to a return to daily life, a reduction in pain and psychological well-being of the patient.
