Factors affecting the price of pharmaceutical products

Abstract:

Tato diplomová práce se zaměřuje na zhodnocení ekonomické návratnosti hrazení biologické léčby u roztroušené sklerózy a ulcerózní kolitidy. Jedná se o onemocnění, pro která je v současné době biologická léčba dostupná, ovšem není z veřejného zdravotního pojištění hrazená všem pacientům. U obou onemocnění dochází vlivem progrese nemoci k postupné invaliditě, ztrátě práceschopnosti a s tím spojenými dopady na ekonomickou situaci pacientů. Cílem této diplomové práce bylo na základě údajů z veřejných registrů a zdravotních pojišťoven statisticky vyhodnotit, zda je biologická léčba úspěšnější v oddálení nástupu invalidity než konvenční terapie. Bylo dokázáno, že biologická terapie vede k výraznějšímu zachování ekonomické aktivity pacientů ve srovnání s pacienty s konvenční léčbou. Dalším cílem práce bylo zhodnotit ekonomickou návratnost hrazení biologické léčby. U onemocnění s rychlou progresí a častým nástupem invalidity, jako je roztroušená skleróza, je pro stát a zdravotní pojišťovny souhrnně ekonomicky výhodnější plošné hrazení biologické terapie. U onemocnění s pomalejším průběhem a nižší mírou invalidity pacientů, jako je ulcerózní kolitida, je ekonomicky nejvýhodnější pro stát a zdravotní pojišťovny hrazení pouze konvenční terapie. S ohledem na průběžný pokles ceny biologické léčby z důvodu nahrazování originálních léčiv generiky je možné v následujících letech očekávat změnu situace. Zejména u onemocnění navozujících invaliditu u velkého podílu pacientů se dá předpokládat, že se v budoucnu stane hrazení biologické terapie ekonomicky výhodnější než hrazení konvenční terapie.

Abstract:

This diploma thesis focuses on the evaluation of the profitability of the reimbursement of biological treatment in multiple sclerosis and ulcerative colitis. Biological treatment is currently available for these diseases, but it is not covered by the public health insurance for all patients. In case of both diseases, the progression of the disease leads to gradual disability, loss of ability to work and the related effects on the patients’ economic situation. The aim of this thesis was to statistically evaluate, based on the data from public registers and data provided by health insurance companies, whether biological treatment is more successful in delaying the onset of disability than conventional therapy. It has been proven that biological therapy leads to a more significant preservation of economic activity of patients compared to conventional treatment. Another goal of the thesis was to evaluate the economic return of paying for biological treatment. In case of diseases with rapid progression and frequent onset of disability, such as multiple sclerosis, overall reimbursement of biological therapy is economically more advantageous for the state and health insurance companies. For diseases with a slower course and a lower degree of patient disability, such as ulcerative colitis, it is economically most advantageous for the state and health insurance companies to cover only conventional therapy. With regard to the continuous decrease in the price of biological treatment due to the replacement of original drugs by generics, it is possible to expect a change in the situation in the following years. Especially for diseases that cause disability in large proportion of patients, it can be assumed that in the future the reimbursement of biological therapy will become more economically advantageous than the reimbursement of conventional therapy.
