Studying asymmetry of the human craniofacial region

Abstract:

Předmětem diplomové práce bylo studium kraniofaciální asymetrie a vliv chi-rurgických korekcí na asymetrii obličeje i lebky s faciální anomálií. Samotné studium asymetrie u jedinců s faciálními anomáliemi má v dnešní době široké uplatnění v mnoha odvětvích, zejména v maxilofaciální chirurgii. Materiál v práci představovaly CBCT a 3D povrchové skeny obličejů dospělých jedin-ců před a po ortognátním/maxilofaciálním zákroku. Dále materiál obsahoval 3D po-vrchové skeny obličejů jedinců zdravé populace z databáze FIDENTIS. Data byla zpra-cována ve virtuálním prostředí počítače a asymetrie obličejů i lebek byla hodnocena pomocí digitalizovaných význačných bodů. Body byly, jako trojrozměrné kartézské souřadnice, použity pro analýzu asymetrie velikosti a tvaru obličejů a lebek pomocí geometrické morfometrie a vícerozměrné statistiky. Asymetrie ve velikosti polovin obličejů byla zjištěna jen párovým testováním u mužů před zákrokem s většími hodnotami velikosti centroidů napravo. U tvarové asymetrie byl prokázán výskyt direkcionální asymetrie u obličejů i lebek, kromě sku-piny lebek před zákrokem. Asymetrie byla nejvýraznější na dolní čelisti a nose. Dále byl zjištěn trend (statisticky nevýznamný) podobnosti tvarové asymetrie u obličejů po zákroku s referencí, zatímco skupiny před a po zákroku si byly nejvzdálenější. Na-konec byla zjištěna i asociace asymetrie mezi obličeji a lebkami, a to zejména před zákrokem.

Abstract:

The aim of this thesis was to study the craniofacial asymmetry and the impact of surgical correction on facial and skull asymmetry affected by facial anomalies. Nowadays, studying the facial asymmetry plays a great role in many domains, espe-cially in maxillofacial surgery. The material consisted of CBCT and 3D surface scans of adult faces conducted before and after an orthognathic/maxillofacial surgery. The material also contained 3D sur-face scans of faces of healthy population originating in the FIDENTIS 3D Face Data-base. The data were processed using virtual environment of a PC and the asymmetry of the faces and skulls was evaluated by digitization of landmarks. The landmarks were used as 3-dimensional cartesian coordinates in size and shape analysis utilizing methods of geometric morphology and multivariate statistic. The asymmetry in size of the halves of the faces was observed only by utiliz-ing paired testing in men before the surgery leading to greater centroid sizes on the right side. In the shape analysis, the directional asymmetry was observed in faces and skulls together except the sub-group of skulls before the surgery. The asymmetry was the most significant in the area of nose and lower jaw. Furthermore, a trend (statisti-cally not significant) of similarity in facial shape asymmetry between the groups “after surgery” and “reference” was observed, while between the groups “before” and “after surgery” was the level not remarkable. The association between facial and skull asymmetry was observed, especially before the surgery.
