Prevalence infekčních komplikací u těžce popálených pacientů: výsledky multicentrické evropské studie

Prevalence of infectious complications in severely burned patients: a multicentre European study results

Abstract:

Úvod: V posledních letech prodělává Evropská popáleninová asociace (EBA) dramatický vývoj. Jedním z nejdůležitějších bodů této transformace je kontrola výskytu infekčních komplikací u popálených pacientů a hodnocení rezistence jednotlivých mikroorganismů. Materiál a metodika: V databázi EBA vybráno 87 center jak v západní tak i ve východní Evropě. Pro diagnostiku infekčních komplikací u pacientů s termickým traumatem, byla použita kriteria ABA (Americká popáleninová asociace). Získaná data byla statisticky zpracována.Výsledky: Celkem bylo zařazeno 134 pacientů (44 žen). Průměrný věk pacientů v souboru byl 40,39 let (SD±22,17). Průměrné ABSI bylo 7,5 (SD±2,54), průměrný rozsah popálené plochy byl 30,49% TBSA (SD±20,14). Průměrná délka hospitalizace v den provedení studie byla 24,32 dní (SD±30,64). Infekční komplikace byly zjištěny u 92 pacientů (68,7%). Celkem 76 pacientů (56,7%) splňovalo kritéria pro infekci popálené plochy, 26 pacientů (19,4%) pro infekci krevního řečiště, 21 pacientů (15,7%) pro pneumonii, 13 pacientů (9,7%) pro infekci močového systému. U 29 pacientů (21,6%) byla zjištěna multifokální infekce. U 67 pacientů (50,0%) byl zjištěn jako potenciálně patogenní mikroorganismus grampozitivní kmen bakterie, u 73 pacientů (54,5%) gramnegativní kmen bakterie a u 18 pacientů (13,4%) byla zjištěna kvasinka. Závěr: Jedním ze základních pilířů budoucích standardů v jednotlivých popáleninových centrech je monitorace výskytu infekčních komplikací u popálených pacientů, prevence šíření těchto komplikací a hlášení rezistentních patogenů v Evropě. Předkládaná práce je důležitým projektem právě v této oblasti.

Abstract:

Introduction: Recently, European Burns Association (EBA) has undergone dramatic development. One of the most important points of this transformation is to control the incidence of infectious complications in burn patients and evaluation of resistance of microorganisms. Material and Methods: In the EBA database we selected 87 burn centers in both, Western and Eastern Europe. For the diagnosis of infectious complications in patients with thermal trauma was used the ABA criteria (American Burn Association). The data were statistically processed. Results: We included 134 patients (44 women). The average age of the group was 40.39 years (SD±22.17). Average ABSI was 7.5 (SD±2.54), the average extent of the burned area was 30.49 % TBSA (SD±20.14). The average length of hospitalization on the day of the study was 24.32 days (SD±30.64). Infectious complications were observed in 92 patients (68.7%). Overall 76 patients (56.7%) met the criteria for infection of the burned area, 26 patients (19.4%) for bloodstream infections, 21 patients (15.7%) for pneumonia, 13 patients (9.7%) for infection of urinary system. 29 patients (21.6%) were found multifocal infection. In case of 67 patients (50.0%) potentially pathogenic gram-positive bacterial strains were found, in case of 73 patients (54.5%) there were gram-negative bacterial strains, and 18 patients (13.4%) were found to yeast. Conclusion: One of the basic pillars of future standards in individual burn centers is monitoring the occurrence of infectious complications in burn patients, prevention of complications and spreading the message of resistant pathogens in Europe. This work is an important project in this field.
