A self-threat: Unanticipated consequences of discoursive policies in drug treatment institutions on clients' abstinence strategies

Abstract:

The initial survey for the present research pointed out the criticality and, at the same time, vagueness of the concept of a loss of control in the diagnosis of addiction and the unsustainable abstinence strategies, which were practised by the pilot respondents immediately after their return from the addiction treatment institutes. The thesis therefore examines the unintended consequences of discursive politics is told to the clients of these institutes about their condition) on the development of their abstinence strategies in the following research steps: 1. What contexts constituted the diagnosis of the loss of control in the respondents’ addiction treatment narratives? 2. What kinds of conceptualisation of their addiction can be found in their narratives? Which of these conceptualisations can be linked to the institutional addiction treatment? 3. How did they define the risk of abstinence persistence and subsequently developed strategies to eliminate or reduce this risk based on these discursive sources? The main source of the data was four interviews with respondents focused on their withdrawal narratives, which were analysed by the tools of discursive analysis. At the theoretical level, the thesis shows how the framing of the respondents’ behaviour by the loss of control in combination with the framing of addiction as a personality trait forms an “possessed” interpretation of an addict who can lose control any time. The practical consequence of this issue was the subsequent respondents’ creation of such strategies, which tried to eliminate any risk and were unsustainable on a long-term basis. As to the recommendations for a more effective practice of addiction treatment institutes, it is important to mention especially a more fine-grained categorization of patients’ problems, sufficient economic and legal assistance, elimination of the “possessed” interpretation of the addiction and the controlled management of risks.

Abstract:

Prvotný prieskum k tomuto výskumu poukázal na kľúčovosť a zároveň vágnosť konceptu straty kontroly v diagnostike závislosti a na neudržateľné stratégie abstinencie, ktoré pilotný respondenti praktikovali bezprostredne po návrate z odvykacích inštitúcií. Práca preto skúma, aké nezamýšľané dôsledky produkujú diskurzívne politiky (to, čo sa klientom týchto inštitúcií hovorí o ich stave) na tvorbu ich stratégií abstinencie a to v nasledujúcich výskumných krokoch: 1. Aké kontexty konštituovali diagnostiku straty kontroly v príbehoch odvykania respondentov? 2. Aké konceptualizácie svojej závislosti možno nájsť v ich rozprávaniach? Ktoré z týchto konceptualizácií možno prepojiť s inštitucionálnou liečbou? 3. Ako na základe týchto diskurzívnych zdrojov definovali riziko pre udržanie abstinencie a následne vytvárali stratégie eliminácie či redukcie tohto rizika? Hlavným zdrojom dát boli štyri rozhovory s respondentmi zamerané na ich príbehy odvykania analyzované nástrojmi diskurzívnej analýzy. V teoretickej rovine práca ukázala, ako rámcovanie správania respondentov stratou kontroly v kombinácii s rámcovaním závislosti ako osobnostnej črty tvorí "posadnutý" výklad závislého, ktorý môže kedykoľvek stratiť kontrolu. Praktickým dôsledkom tohto problému bola následne tvorba stratégií respondentmi, ktoré sa pokúšali úplne eliminovať akékoľvek riziko a boli dlhodobo neudržateľné. Doporučenia pre efektívnejšiu prax odvykacích inštitúcií smerujú najmä na jemnejšiu kategorizáciu problémov pacientov, dostatočnú ekonomickú a právnu pomoc, odstránenie "posadnutého" výkladu závislosti a kontrolované čelenie rizikám.
