Physiotherapy for adults with cerebral palsy

Abstract:

Souhrn: Název: Fyzioterapie u dospělých s DMO Cíl: Cílem práce je přispět k řešení problematiky volby optimálních fyzioterapeutických postupů, metod a konceptů u dospělého klienta s DMO Metodika: K vstupnímu vyšetření vybrané dospělé klientky s DMO jsem použila odběr anamnézy, kineziologický rozbor, antropometrické vyšetření, určení lokomoční úrovně, vyšetření hybnosti a svalového tonu, spasticity ( použitím Ashwortovy škály) a neurologické vyšetření fyzioterapeutem ; při ergoterapeutickém vyšetření jsem určila míru soběstačnosti testem podle Barthelové a vyšetřila funkční úchop ruky. Na základě kineziologického rozboru jsem vypracovala šestiměsíční rehabilitační plán. Jako vhodnou motivaci ke cvičení jsem použila strukturalizaci podle metodiky práce s autistickými klienty, rytmizaci a hudbu. K celkovému snížení spasticity před zahájením cvičební jednotky jsem zvolila kromě vodoléčby práci s úvodní polohou podle Jílkové a reflexní terapii plosek. Spasticitu při cvičení jsme ovlivnili použitím konceptu Spiraldynamic? , delší výdrží v jednotlivých polohách a prohloubeným dýcháním. K ovlivnění patologického postavení osy, decentrace kloubů a stimulace hlubokého stabilizačního systému jsme využili Vojtovu metodu reflexní lokomoce, Cvičení podle Jílkové, Spiraldynamic?, nácvik stoje a chůze u rámu dveří a polohování při ADL. Spiraldynamic? koncept jsem vybrala i k podpoře stabilizační funkce vleže na boku. Klientka polohovala ve vertikalizační bedně. Výsledky: Začátkem prosince 2015 došlo ke zhoršení emoční lability klientky. Z toho důvodu byla navýšena medikace. To však vedlo k celkovému zvýšení spasticity. Nástup spasticity ovlivnil zhoršení spontánní posturální situace klientky, konkrétně postavení pravého kyčelního kloubu ve smyslu VR a ADD, ale i osy trupu. Zvýraznila se rotace hrudníku do leva a dopředu, skolióza i držení ramen v protrakci. I ve výstupním vyšetření ještě převažoval tento obraz. Při fyzioterapii se ale dařilo patologii ovlivnit (zvlášť za přítomnosti více terapeutů při cvičení). Při cvičební jednotce se snížila spasticita na DK a uvolnily oba kyčelní klouby. Díky zvětšenému rozsahu ZR a ABD v kyčelních kloubech bylo možné lépe nastavit výchozí cvičební polohy. Následně při použití jednotlivých metodik docházelo k centraci kořenových kloubů. V nácviku sedu klientka symetricky zatížila sedací hrboly. Během vertikalizace u rámu dveří zapojila hluboký stabilizační systém při nácviku stoje. Při nácviku stabilizační funkce vleže na boku se podařilo naučit klientku aktivnímu pohybu při zevní žeberní osmičce metodou Spiraldynamic?. Po ukončení cvičební jednotky a polohování se ale opět posturální situace vracela do popsaného patologického nastavení ovlivněného zvýšenou spasticitou .

Abstract:

Title: Physiotherapy in adults with cerebral palsy. Objective: The aim of this thesis is to contribute to solving problems of optimal choice of physiotherapy procedures, methods and concepts for an adult client with cerebral palsy. Methodology: For the entrance examination of selected adult client with CP, I used the patient's medical history, kinesiology analysis, anthropometric tests, determining the locomotion level, examinations of mobility and muscle tone, spasticity (using Ashwort scale) and neurological examination by a physiotherapist; during the occupational therapy examination, I determined the degree of autonomy test by Barthel and investigated the functional grip hand. For my client I developed for six months rehabilitation plan based on kinesiology analysis. As appropriate motivation to exercise, I used the structuring according to the methodology of work with autistic clients, and also rhythm and music. To overall reduction of spasticity before beginning an exercise unit, I chose besides hydrotherapy also work with the introductory position by Jílková and reflexology soles. Spasticity during exercise we have affected applying the concept Spiraldynamic?, longer staying in various postures and deep breathing.To influence the pathological status of the axis, decentralization joints and stimulating deep stabilization system, we used Vojta reflex locomotion method, exercises according Jílková, Spiraldynamic?, practice standing and walking in the door frame and positioning with ADL.Spiraldynamic? concept I chose also to support the stabilizing function in lying on the side. To train verticalization position the client used the positioning in the verticalization box. Results: In early December 2015 worsened the client´s emotional lability. For this reason, we increased medication. This lead to an overall increase in spasticity. The onset of spasticity negatively influenced the position of right hip in terms of VR and ADD, but also the axis of the trunk. Highlighted the rotation of the chest to the left and forward, scoliosis and holding of arms in protraction. Even in the exit examinations have prevailed this image. During physical therapy, however, we managed to influence the pathology (particularly in the presence of multiple therapists during exercise. Due to the increased scope and ZR ABD in the hip joints we can better set the default exercise positions. Subsequently, using different methodologies, occurred centration root joints. In practicing sitting client symmetrically burdened buttock. During verticalization in the door frame she engaged the deep stabilizing system while practicing standing. During training stabilizing function of the lying on her side we managed to learn the client active movement in the outer rib Eight method Spiraldynamic?. But after completion of exercises and positioning the postural situation again returned to described pathological settings affected by increased spasticity. There was an improvement of intestinal peristalsis and problems with constipation subsided. Klient very well accepted removable abduction wedge, which improved sitting position with symmetrical load sofas bumps in the truck. Position of acre of toes was well corrected with orthopedic shoe korfant inserts. There have been improvements in tolerance of exercise unit. The client perceived exercise positively even in periods of heightened emotional lability.
