Abstract:

Cíl: Hlavním cílem předkládané diplomové práce bylo zmapovat úroveň znalostí, názorů a kompetencí všeobecných sester pracujících v intenzivní medicíně v managementu léčby pooperační bolesti a posouzení existence a činnosti APS ve vybraných fakultních nemocnicích. Dílčí cíle předkládají primární souhrn informací týkající se problematiky managementu léčby pooperační bolesti, existenci a činnost APS, posuzují úroveň vědomostí a vliv získaného vzdělání všeobecných sester pracujících v intenzivní medicíně vztahujících se k otázkám managementu léčby pooperační bolesti a hodnotí eventuality a způsoby edukace všeobecných sester v managementu léčby pooperační bolesti Teoretická část sumarizuje aktuální situaci managementu léčby pooperační bolesti v ČR a v zahraničí ve světle současné medicíny Praktická část mapuje úroveň znalostí, kompetencí a názorů všeobecných sester pracujících v intenzivní medicíně v oblasti organizace léčby pooperační bolesti v závislosti na délce praxe, na činnosti APS, na metodách edukace a na dosaženém stupni vzdělání Metodika: Byla zvolena forma dotazníkového šetření, kterého se účastnilo 196 všeobecných sester pracujících v intenzivní medicíně ve vybraných fakultních nemocnicích (FN Olomouc, FN Ostrava, FN Hradec Králové a FN u sv. Anny v Brně). Při získávání dat byl použit nestandardizovaný, polootevřený a anonymní dotazník s předem formulovanými otázkami. Dotazníky byly distribuovány v průběhu měsíců březen ? listopad 2014. Získaná data byly registrována, následně analyzována a statisticky vyhodnocena. Závěr: Bylo potvrzeno zavedení APS ve většině nemocnic fakultního typu, kde probíhalo výzkumné šetření, a zároveň byla zmapována úroveň znalostí všeobecných sester. Provedení tohoto prvotního mapování ukázalo dostatečnou znalost sester léčby pooperační a akutní bolesti, bez ohledu na implementaci APS. Současně ukázalo na rezervy, které ve znalostech problematiky všeobecné sestry pracující v intenzivní medicíně mají. Výsledky této práce se dají použít nejen při plánování edukace personálu, ale i při dalším ověřovacím výzkumu.

Abstract:

Objective: The main aim of the thesis was to find out the level of knowledge, attitudes and skills of intensive care nurses in the treatment of postoperative pain management and the assessment of existence and activity of APS in selected teaching hospitals. Sub-objectives presented a primary summary of the information regarding the management of postoperative pain treatment, the existence and activity of APS, assess the knowledge level and the impact on the training of intensive care nurses working with the issues related to the management and treatment of postoperative pain, and evaluate contingencies and methods of education of nurses in the management of postoperative pain treatment. Theoretical part summarizes the current situation of the management of postoperative pain treatment in the Czech Republic and abroad in the light of contemporary medicine. Practical part describes the level of knowledge, skills and opinions of intensive care nurses working in the organization of postoperative pain treatment, depending on the length of practice, the APS activities, on education methods and the achieved level of education. Methods: We chose the form of questionnaire, it was filled out by 196 intensive care nurses working in selected teaching hospitals (University Hospital Olomouc, Ostrava University Hospital, Hradec Králové FN and FN St. Anne's University). To obtain the data a non-standardized, semi-open and anonymous questionnaire with pre-formulated questions was used. Questionnaires were distributed during the months of March to November 2014. The data were registered, analyzed and statistically evaluated. Conclusion: The introduction of APS has been confirmed in most of the University hospitals, where the research took place, and the level of knowledge of nurses was mapped at the same time. The implementation of this initial mapping showed sufficient knowledge of nurses and treatment of postoperative acute pain, regardless of the implementation of APS. At the same time, it showed the lack off familiarity with the discussed issues that the intensive medicine nurses have. The results of this work can be used not only in the planning of staff education, but also in further validation research.
