Genetic predispositions to periodontal diseases and their relationship to systemic diseases

Abstract:

Parodontitida je chronické komplexní zánětlivé onemocnění bakteriálního původu, které vede k destrukci periodoncia a okolních struktur. Cílem práce byla identifikace genových polymorfismů a dalších parametrů spojených s vyšším rizikem vzniku a rozvoje parodontitidy a její vztah k systémovým onemocněním, jako je diabetes mellitus (DM). Práce je komentovaným souborem čtyř vědeckých prací uchazečky. V první práci jsme prokázali, že plasmatické hladiny interleukinu-8 (IL-8) jsou významně odlišné mezi zdravými kontrolami a pacienty s parodontitidou a DM 1. typu (DM1)/DM 2. typu (DM2), (P < 0,01). U pacientů pouze s parodontitidou byly tyto hladiny výrazně nižší než u pacientů s parodontitidou a současně diabetem (P ≤ 0,05). Nezjistili jsme však žádný statisticky významný vztah polymorfismů IL-8 (–251A/T) nebo CXCR2 (+1208C/T) k plasmatickým hladinám IL-8 a k přítomnosti bakterií. V další studii jsme analyzovali 3 polymorfismy (+785C/T), (+1208T/C) a (+1440A/G) v genu CXCR2, receptoru pro IL-8, a nenašli jsme signifikantní rozdíly v jejich frekvencích mezi pacienty s parodontitidou a zdravými kontrolami (P > 0,05). U mužů s alelou C polymorfismu CXCR2 (+785C/T) a alelou T polymorfismu CXCR2 (+1208C/T) jsme však zjistili vyšší frekvenci výskytu bakterie Aggregatibacter actinomycetemcomitans subgingiválně (P < 0,05). V práci zabývající se adipokiny jsme prokázali, že hladiny adiponektinu (ADIPOQ), leptinu (LEP) a resistinu (RETN) v plasmě byly u kontrol a případů podobné a bez závislosti na genové variabilitě. Frekvence polymorfismu ADIPOQ (+45G/T) se lišila u zdravých jedinců a pacientů s parodontitidou; nositelé genotypu TT měli nižší riziko rozvoje parodontitidy než ti s genotypy TG a GG (P = 0,03). Prokázali jsme tedy, že parodontitida neovlivňuje plasmatické hladiny IL-8, ale že pacienti s parodontitidou a diabetem mají vyšší hladiny IL-8, než zdravé kontroly a nediabetici s parodontitidou. Také jsme zjistili, že polymorfismy v genu pro CXCR2 mohou být u českých mužů trpících parodontitidou spojeny s významnější subgingivální kolonizací G- bakteriemi. Polymorfismus ADIPOQ +45G/T může být považován za jeden z faktorů podílejících se na rozvoji parodontitidy.

Abstract:

Periodontitis is a chronic complex inflammatory disease of bacterial origin which leads to destruction of the periodontium and surrounding structures. The aim of this work was to identify gene polymorphisms and other parameters associated with a higher risk of developing periodontitis, and its relationship to systemic diseases, such as diabetes mellitus (DM). This work is a commentary on four original scientific papers of the candidate. In the first work, we found that Interleukin-8 (IL-8) plasma levels differ significantly between healthy controls and periodontal patients with Type 1 diabetes (DM1)/Type 2 diabetes (DM2), (P < 0.01). These plasma levels were significantly lower in patients with periodontitis alone than in periodontal patients with diabetes (P ≤ 0.05). We did not find any statistically significant association between the polymorphisms IL-8 (–251A/T) or CXCR2 (+1208C/T) with the IL-8 plasma levels or with the presence of bacteria. In the next study we analysed 3 polymorphisms (+785C/T), (+1208T/C) and (+1440A/G) in the CXCR2 gene, the IL-8 receptor, and we found no significant differences between the frequency of these polymorphisms in patients with periodontitis and healthy controls (P > 0.05). In men with the presence of the C allele in the CXCR2 (+ 785C/T) polymorphism and the T allele in the CXCR2 (+1208C/T) polymorphism we found a higher frequency of Aggregatibacter actinomycetemcomitans subgingivally (P < 0.05). In the work of adipokines, we found that plasma levels of adiponectin (ADIPOQ), leptin (LEP) and resistin (RETN) did not differ between cases and controls and were not dependent on the gene variability. The frequency of the ADIPOQ (+45G/T) polymorphism varied between healthy subjects and the periodontal patients; the TT genotype carriers have a lower risk of developing periodontitis than those with the TG and GG genotypes (P = 0.03). So, we have shown that periodontitis does not affect IL-8 plasma levels and that patients with periodontitis and diabetes have higher IL-8 levels than healthy controls and non-diabetic patients with periodontitis. We have also shown that polymorphisms in the CXCR2 gene may be associated with significant subgingival colonisation of G-bacteria in Czech men suffering from periodontitis. The ADIPOQ (+45G/T) polymorphism can be considered as one of the factors involved in development of periodontitis.
