Constitutional application of occupational therapy in a diagnostic facility for youth

Abstract:

Abstrakt Úvod: Bakalářská práce se zabývá možnou aplikací ergoterapie v diagnostickém ústavu pro mládež a zkoumá její potenciál fungování mezi aktuálními výzvami a budoucím uplatněním v životě dospívajících chlapců. Práce je rozdělena na teoretickou a praktickou část. V teoretické části se zaměřuje na legislativní ukotvení instituce, poruchy chování, současné terapeutické přístupy a jejich aplikaci v diagnostickém ústavu pro mládež. Praktická část se věnuje využití ergoterapie u vybraného případu mladistvého. Cíl práce: Cílem této práce je aplikovat ergoterapeutické přístupy s technikami motivace pro budoucí pracovní uplatnění klienta diagnostického zařízení. Metodika: Případová studie se zaměřuje na sedmnáctiletého chlapce s diagnózou hyperkinetické poruchy chování, zařazeného do výchovné skupiny Diagnostického ústavu. Pro vstupní vyšetření byl využit vstupní rozhovor, Montrealský kognitivní test (MoCA) ve verzi CZ2 a aplikace Kanadského hodnocení výkonu zaměstnávání (COPM). Schopnost soběstačnosti byla hodnocena testem Barthelové. Fatické a gnostické funkce byly vyšetřeny orientačně během vstupních rozhovorů. Na základě testu COPM byl sestaven krátkodobý terapeutický plán. Případová studie zahrnovala 12 osobních setkání s probandem, rozložených do 3 měsíců. Ergoterapie zahrnovaly motivační techniky jako SWOT analýzu a techniky vycházející z Maslowovy teorie, dále rozhovory a tréninky kognitivních. Na závěr bylo provedeno výstupní vyšetření a navržena doporučení pro dlouhodobý ergoterapeutický plán. Výsledky: Celý ergoterapeutický proces se podařilo aplikovat a porovnání výsledků vstupního a výstupního ergoterapeutického hodnocení ukázalo, že ergoterapeutické intervence byly účinné. Závěr: Cíle a úkoly práce se podařilo splnit.

Abstract Introduction: This bachelor's thesis examines the possible application of occupational therapy in a diagnostic youth facility and explores its potential to function between current challenges and future applications in the lives of adolescent boys. The thesis is divided into a theoretical and a practical part. The theoretical part focuses on the legislative anchoring of the institution, behavioural disorders, current therapeutic approaches and their application in a diagnostic institute for youth. The practical part focuses on the use of occupational therapy in a selected case of a juvenile. Aim of the thesis: The aim of this thesis is to apply occupational therapy approaches with motivation techniques for the future employment of a client of a diagnostic facility. Methodology: The case study focuses on a 17-year-old boy diagnosed with hyperkinetic behavioural disorder, assigned to the educational group of the Diagnostic Institute. An initial interview, the Montreal Cognitive Assessment (MoCA) version CZ2 and an application of the Canadian Employment Performance Assessment (COPM) were used for the initial assessment. The ability to be self-sufficient was assessed with the Barthel test. Fatal and gnostic functions were examined tentatively during the intake interviews. A short-term treatment plan was developed based on the COPM test. The case study included 12 face-to-face sessions with the proband, spread over 3 months. Occupational therapy included motivational techniques such as SWOT analysis and techniques based on Maslow's theory, as well as interviewing and cognitive training. Finally, an exit examination was conducted and recommendations for a long-term occupational therapy plan were proposed. Results: The entire occupational therapy process was successfully applied and a COPMarison of the results of the initial and outcome occupational therapy assessments showed that the occupational therapy interventions were effective. Conclusion: The aims and objectives of the work were achieved.
