Abstract:

Diabetes a těhotenství. V naší práci jsme prokázali u sledovaných žen s gestačním diabetem signifikantní korelaci mezi hladinami cytokinů TNF- α; IL-6; IL-10 v periferní krvi a systolickým i diastolickým krevním tlakem ve druhém; třetím trimestru. Potvrdili jsme statisticky významné zvýšení rizika preeklampsie pro skupinu high TNF-α ve třetím trimestru a negativní korelaci pro období druhého trimestru mezi porodní váhou a hladinami cytokinů TNF-α; IL-6, IL-10. Výsledky naší práce by mohli pomoci v časné diagnostice rozvoje závažné komplikace gravidity preeklampsie, protože žádný jednotlivý v současné době dostupný parametr nemá dostatečnou sílu průkazu.Skríning hladin cytokinů v periferní krvi (TNF-α; IL-10; IL-6) během druhého trimestru se může zdát být předčasný, je ještě třeba provést cost–benefit analýzy v rámci rozsáhlých randomizovaných klinických studií. V naší studii byla minimální porodní váha 2760 g, tedy žádné z dětí nenaplnilo kritéria pro nízkou porodní hmotnost. Naše výsledky podporují hypotézu, že prozánětlivý stav spojený se zvýšením hladin prozánětlivých cytokinů TNF-α; IL-6 hraje významnou roli ve zvýšení krevního tlaku. Dále jsme prokázali negativní korelaci mezi prozánětlivými cytokiny TNF-α; IL-6 a porodní hmotností.

Abstract:

Diabetes and Pregnancy. In our study we found in observed women with gestational diabetes significant correlation between TNF- α; IL-6; IL-10 levels and both systolic and diastolic blood pressure in the second trimester; the third trimester. We demonstrated a statistically significant positive association between high TNF-α group and preeclampsia risk in the third trimester. We also determined the negative correlation in the second trimester between birth weight and TNF-α; IL-6, IL-10 levels. The results of our study could help in the early diagnosis of the development of a severe pregnancy complication preeclampsia. There is currently no single parameter available that has sufficient power to diagnose this severe complication. It may seem premature to start screening, cost-benefit analysis is still needed and large-scale randomized clinical trials must be performed. In our study, the minimum birth weight was 2760 g, ie none of the children fulfilled the criteria for low birth weight. Our results support the hypothesis that the pro-inflammatory status associated with increased levels of pro-inflammatory cytokines TNF-α; IL-6 plays an important role in pathogenesis of elevated blood pressure. Furthermore, we demonstrated a negative correlation between pro-inflammatory cytokines TNF-α; IL-6 and birth weight.
