Evaluation of gross motor function in cerebral palsied children

Abstract:

Tato práce se zabývá problematikou dětské mozkové obrny (DMO). U skupiny DMO dětí (n=16) ročníků narození 2010 a 2011, které jsou v péči regionálních rehabilitačních ambulancí, bylo provedeno testování jejich hrubých motorických funkcí s využitím testovacího nástroje Gross Motor Function Measure-66 (GMFM-66). Každé dítě rovněž bylo s využitím definovaného a ve světě respektovaného klasifikačního systému hrubé motoriky Gross Motor Function Classification System (GMFCS) zařazeno do jednoho z pěti stupňů GMFCS (I - nejlehčí míra hrubě motorického deficitu, V - nejtěžší míra). Výsledky testování byly vyhodnoceny pomocí počítačového softwaru Gross Motor Ability Estimator (GMAE-2). Získaná data byla statisticky vyhodnocena. Hlavním cílem diplomové práce bylo zhodnocení, zda se naměřené hodnoty GMFM-66 pohybují dle standardizovaných parametrů motorického vývoje v rámci GMFCS úrovně v 50% rozsahu hodnot od limitu GMFM-66 (norma). Tento 50% rozsah je připouštěn jako hodnota, kdy se děti v GMFCS úrovni pohybují stále v okolí normy. Tyto standardizované parametry vycházejí z křivek hrubě motorických funkcí, které jsou výsledkem rozsáhlé studie populace jedinců s DMO v Kanadě. Tyto standardizované křivky dokládají vývoj hrubě motorických schopností v průběhu života dítěte s DMO. Křivky obsahují fázi růstu hrubé motoriky i fázi plateau, kdy v závislosti na GMFCS úrovni a věku dítěte již funkční kapacita DMO jedince neroste. Ve světě jsou křivky využívány nejen jako nástroj prognostický, ale rovněž jsou používány jako nástroj k určení, zda má rehabilitační léčba pozitivní vliv na hrubé motorické funkce nebo zda má být rehabilitační péče hrazena z prostředků veřejného zdravotního pojištění. Doplňkovým výzkumem práce bylo anketní šetření "Rehabilitace v průběhu let". Cílem tohoto doplňkového výzkumu bylo zjistit zahájení a frekvenci rehabilitace u probandů do dvou let věku, kdy je rehabilitace u DMO považována za nejefektivnější. Výsledky z anketního šetření byly statisticky zpracovány a dány do korelace s výsledky GMFM-66. Z výsledků práce je zřejmé, že se skóre GMFM-66 pohybují v okolí 50% rozsahu od limitu GMFM-66 v rámci GMFCS úrovní. Začátek a frekvence rehabilitace nemá na dosažené skóre GMFM-66 vliv.

Abstract:

This thesis deals with cerebral palsy (CP) issues. For a CP group of children (n=16), born in years 2010 and 2011, which are under the care of regional rehabilitation outpatients, their gross motor functions were tested using a test tool Gross Motor Function Measure-66 (GMFM-66). Each child was stratified by classification system Gross Motor Classification System included in one of five levels (I- the lightest degree of gross motor deficit, V- the hardest form). The test results were evaluated using computer software Gross Motor Ability Estimator (GMAE-2). The data obtained were statistically evaluated. The goal was whether the measured GMFM-66 values are within the GMFCS level within 50% of the range of GMFM-66 limits (norm). This 50% range is acceptable as the value at which GMFCS children are still around the standard. These standardized parameters are based on gross motor function curves resulting from a large population study of CP population in Canada. These standardized curves document the development of gross motor abilities during the life of a CP child. The curves contain the stage of gross motor growth and the plateau stage, when depending on the GMFCS level and age of the child, the functional capacity of an individual CP does not increase. In the world, curves are used not only as a prognostic tool but are also used as a tool to determine whether rehabilitation treatment has a positive effect on gross motor function or whether rehabilitation care will be covered by public health insurance. A complementary research of the thesis was the survey "Rehabilitation over the years". The aim of this complementary research was to find out the frequency and initiation of rehabilitation in probands up to two years of age when rehabilitation at CP was considered the most effective. The survey results were statistically processed and correlated with the GMFM-66 results. From the results of the thesis, it is clear that the GMFM-66 scores are around 50% of the GMFM-66 range within the GMFCS levels. The start and frequency of rehabilitation does not affect the achieved GMFM-66 score.
