Abstract:

Cíl: Hlavním cílem výzkumného šetření bylo zhodnotit dodržování kritérií během weaningu čili ukončování UPV (umělá plicní ventilace) u pacientů v intenzivní péči. Mezi dílčí cíle patřilo srovnat délku UPV, délku hospitalizace na intenzivním lůžku, srovnat počet extubací/dekanylací (vytažení endotracheální/trachestomické kanyly) a srovnat počet komplikací, zejm. reintubace/rekanylace (znovuzavedení endotracheální/trachestomické kanyly) mezi skupinou, u které probíhalo odpojování od UPV s implementovaným Weaning protokolem (1. skupina - protokolizovaná) se skupinou, u které probíhalo odpojování od UPV bez weaning protokolu (2. skupina - kontrolní). Metodika: Za účelem výzkumného šetření byl vytvořen "Weaning protokol" jako praktický manuál pro odpojování od UPV. Informace pro tvorbu weaning protokolu byly čerpány z českých i zahraničních zdrojů, vyhledávány prostřednictvím systémů a přístupných databází (MEDVIK, Google Books, Google Scholar, CINAHL, EBSCO Publishing, MEDLINE, PubMed). Validovaný Weaning protokol je rozdělený na 3 části. První část zahrnuje zahájení weaningu, na ni navazuje druhá část - provedení SBT a třetí část zahrnuje extubaci/dekanylaci pacienta. Protokol byl následně implementován do praxe na ARO (Anesteziologicko-resuscitační oddělení) Kroměřížské nemocnice a.s. Pro hodnocení dodržování kritérií během weaningu byl sestaven záznamový arch a ověřování, zda byla kritéria Weaning protokolu dodržována, probíhala dotazováním na ošetřující zdravotnický tým (vedoucí lékař, ošetřující lékař, ošetřující sestra). Výzkumný soubor tvořilo 40 pacientů hospitalizovaných na ARO Kroměřížské nemocnice a. s. s nutností UPV v procesu odvykání od UPV až do převedení na spontánní ventilaci s následnou extubací/dekanylací pacienta. Tito pacienti byli randomizací rozděleni na dvě skupiny, tj. skupina 1 - protokolizovaná, skupina 2 - kontrolní (neprotokolizovaná). Následně proběhla komparace outcomes parametrů mezi oběma skupinami, zejm. v délce UPV, délce hospitalizace na intenzivním lůžku, v počtu extubací/dekanylací a v počtu komplikací, zejm. reintubace/rekanylace. Výzkumné šetření bylo započato v květnu 2019 a ukončeno v lednu 2020. Následně byla data statisticky zpracována. Výsledky: Výsledky výzkumného šetření nepoukazují na významnější rozdíly mezi skupinami. Délka UPV u protokolizované skupiny (skupina 1) byla v průměru 214,5 hodin, u kontrolní skupiny (skupina 2) 213,3 hodin (p=0,981). Délka hospitalizace na intenzivním lůžku u protokolizované skupiny byla v průměru 12,7 dne, u kontrolní skupiny 13,7 dne (p= 0,720). Extubováno bylo celkem u 55 % (n=11) pacientů v prokololizované skupině, v kontrolní skupině bylo extubováno 50 % (n=10) pacientů (p=0,624). Dekanylováno bylo celkem 15 % (n=3) pacientů v protokolizované skupině, v kontrolní skupině bylo dekanylováno 25 % (n=5) pacientů (p=0,624). Komplikace v podobě reintubace a rekanylace byly v protokolizované skupině 4, a to 3 reintubace a 1 rekanylace, v kontrolní skupině taktéž 4 a to 3 reintubace a 1 rekanylace (p=1,000). Dodržování kritérií během weaningu bylo hodnoceno kvalitativně a u všech sledovaných pacientů kritéria weaningu byla dodržena.

Abstract:

Objectives: The main objectives of the research survey were to evaluate the compliance of the criteria during weaning or termination of MLV (mechanical lung ventilation) in patients under intensive care. The partial goals included the comparison of the length of MLV, length of hospitalization under intensive care, the contrast of the number of extubations/decannulations, and the comparison of the number of complications, especially reintubation/recannulation (re-insertion of the endotracheal/tracheostomy cannula) among groups disconnected from MLV with implemented Weaning protocol (group 1 - protocoled) group in which the disconnection from MLV took place without the weaning protocol (group 2 - control group). Methodology: For the purpose of the research investigation, the "Weaning Protocol" was created as a practical manual for disconnection from MLV. The information for the creation of the weaning protocol was drawn from the Czech and foreign sources searched through the systems and accessible databases (MEDVIK, Google Books, Google Scholar, CINAHL, EBSCO Publishing, MEDLINE, PubMed). The validated Weaning protocol is divided into three parts. The first part involves initiating weaning, followed by the second part - performing SBT, and the third part involves extubation/decannulation of the patient. The protocol was initially implemented in practice at the ARD (Anesthesiology and Resuscitation Department) of the Hospital in Kroměříž. A record sheet was compiled to assess the compliance with the criteria during weaning, and the verification of whether the criteria of the Weaning Protocol were complied with was conducted by interviewing the nursing medical team (chief physician, attending physician, attending nurse). The research group consisted of 40 patients hospitalized at the ARD of the Hospital in Kroměříž with the need for MLV in the process of weaning from MLV until the conversion to spontaneous ventilation with subsequent extubation/decannulation of the patient. These patients were randomized into two groups, i.e., group 1 - protocoled, group 2 - control group (non protocoled). Subsequently, the parameter outcomes were compared between the two groups, especially in the length of MLV, the length of hospitalization in the intensive care unit, in the number of extubations/decannulations, and in the number of complications, especially reintubation/recannulation. The research survey was started in May 2019 and completed in January 2020. Subsequently, the data were statistically processed. Results: The results of the research survey do not indicate any significant differences between the groups. The duration of MLV in the protocol group (group 1) averaged 214.5 hours in the control group (group 2) 213.3 hours (p=0.981). The length of hospitalization in the intensive care unit in the protocol group was, on average, 12.7 days, in the control group 13.7 days (p=0.720). A total of 55 % (n=11) patients in the protocolized group were extubated, 50 % (n=10) patients (p=0.624) were extubated in the control group. The total of 15 % (n=3) patients in the protocol group were decannulated, 25 % (n=5) patients (p=0.624) were decannulated in the control group. Complications in the form of reintubation and recannulation were in the protocol group 4, namely 3 re-intubations and 1 recannulation, in the control group also 4, namely 3 reintubations and 1 recannulation (p=1,000). The adherence to the criteria during weaning was assessed qualitatively, and the weaning criteria were met in all monitored patients.
