FGF23, the hormone regulating phosphatemia and its importance in nephrological patients and in clinical osteology

Abstract:

Provedli jsme retrospektivní analýzu laboratorních parametrů kalciofosfátového metabolismu dialyzačního oddělení Klatovské nemocnice, a.s. za období únor 2019 až říjen 2022. Bylo vyšetřeno 163 pacientů stádia G4 a G5, celkem 924 odběrů. 96 % z toho ve stádiu G5. Byly analyzovány sérové parametry vápník, albumin, fosfor, kreatinin, 1-84 PTH, FGF23, 25-OHD a 1,25-(OH)2D pomocí biochemických (analyzátor Architect ci8200, Abbott, USA) a imunoanalytických metod na analyzátoru LIAISON XL, DiaSorin, Italy. Mezi stádii G4 a G5 jsme nalezli statisticky významné vyšší hodnoty u G5 v FGF23, fosforu a kreatininu. U stádia G5 spolu pozitivně, středně významně, korelovaly hodnoty FGF23 a hladiny fosforu (r = 0,659 a p větší než 0,0001). Pouze 23,9 % odběrů ve stádiu G5 mělo dostatečnou hladinu vitaminu D. Naopak 32,81 % měření mělo hladinu 25-OHD deficitní. Hodnoty FGF23 (p 0,0001), 1-84 PTH (p větší než 0,0001) a kreatininu (p větší než 0,0001) se statisticky významně lišily dle kategorie hladiny fosforu (hypofosfatémie, normofosfatémie a hyperfosfatémie). Nejnižší hodnoty byly u hypofosfatémie a nejvyšší u hyperfosfatémie). Hodnoty 1-84 PTH statisticky významně rostly dle kategorie korigovaného vápníku (hypokalcémie, normokalcémie a hyperkalcémie) (p větší než 0,0001). Pouze 23,4 % měření u stádia G5 vykazovalo současně normofosfatémii i normokalcémii, což bylo signifikantně méně než u stádia G4. Během tříletého sledování pacientů ve stádiu G5 dochází k postupnému vzestupu hodnot korigovaného vápníku, poklesu 1-84 PTH, vzestupu FGF23, zatímco hladiny fosforu nevykazovaly žádný významný trend v čase.

Abstract:

We performed a retrospective analysis of laboratory parameters od calcium-phosphate metabolism in the dialysis department of Klatovy Hospital, a.s., for the period February 2019 to October 2022. 163 patients of G4 and G5 stage were examined, a total of 924 samples were taken. Serum parameters calcium, albumin, phosphorus, creatinine, 1-84 PTH, FGF23, 25-OHD and 1,25-(OH)2D were analyzed using biochemical (Architect ci8200 analyzer, Abbott, USA) and immunoanalytical methods on a LIAISON XL analyzer, DiaSorin, Italy. Between G4 and G5 stages, we found statistically significant higher values for G5 in FGF23, phosphorus and creatinine. At stage G5, FGF23 and phosphorus levels were positively correlated with each other, with moderate signifikance (r = 0,659 and p higher than 0,0001). Only 23,9 % of G5 stage samples had sufficient vitamin D levels. In contrast, 32,81 % of the measurements had 25-OHD deficient levels. The values of FGF23 (p higher than 0,0001), 1-84 PTH (p higher than 0,0001) and creatinine (p higher than 0,0001) were statistically significantly different according to the category of phosphorus level (hypophosphatemia, normophosphatemia and hyperphosphatemia) The lowest values were in hypophosphatemia and the highest values in hyperphosphatemia. The 1-84 PTH values increased statistically significantly according to the category of corrected calcium (hypocalcemia, normocalcemia and hypercalcemia) (p higher than 0,0001). Only 23,4 % of the measurements at stage G5 showed both normophosphatemia and normocalcemia, which was significantly less than at stage G4. During the 3 year follow-up of stage G5 patients, there was a gradual increase in corrected calcium levels, a decrease in 1-84 PTH, and an increase in FGF23, while phosphorus levels showed no significant trend over time.
