Abstract:

BURIANOVÁ, Jitka. Specifika ošetřovatelské péče o pacienty s mechanickou srdeční podporou HeartMate 3. Vysoká škola zdravotnická, o. p. s. Stupeň kvalifikace: Bakalář (Bc.). Vedoucí práce: doc. MUDr. Ondrej Szárszoi, Ph.D. Praha. 2023. 70 s. Tématem bakalářské práce jsou specifika ošetřovatelské péče o pacienty s mechanickou srdeční podporou HeartMate 3. Hlavním cílem je zaměřit se na ošetřovatelskou péči u pacienta před a po implantování mechanické srdeční podpory HeartMate 3. Teoretická část se zabývá v první kapitole problematikou srdečního selhání, jeho dělením, etiologií, prognózou a následně i terapií srdečního selhání. Druhá kapitola teoretické části je věnována mechanickým srdečním podporám, kde více rozepisujeme klasifikaci mechanických srdečních podpor, jejich taktiku a cíle využití, indikace i kontraindikace implantace podpory a také se věnujeme chirurgickým aspektům implantace této specifické podpory. Ve třetí kapitole jsou popsána specifika ošetřovatelské péče o pacienty s HeartMate 3 a to jak v předoperačním, intraoperačním i pooperačním obdobím. Praktická východiska naší diplomové práce popisují ošetřovatelský proces, jeho fáze a konkrétní kazuistiku a ošetřovatelský proces u pacientky, u které byla indikována implantace mechanické srdeční podpory HeartMate 3. Pro zpracování dat praktické části byl využit model 13 funkčních vzorců zdraví Marjory Gordonové. V závěru diplomové práce jsme navrhli doporučení pro praxi, která jsou odkázána pro zdravotní personál, pro samotného pacienta, ale i pro jeho rodinu.

MD Ondrej Szárszoi, Ph.D. Prague. 2023. 70 p. The topic of the bachelor's thesis is the specifics of nursing care for patients with HeartMate 3 mechanical cardiac support. The main goal is to focus on nursing care for the patient before and after the HeartMate 3 mechanical cardiac support is implanted. In the first chapter, the theoretical part deals with the issue of heart failure, its division, etiology, prognosis and, subsequently, the therapy of heart failure. The second chapter of the theoretical part is dedicated to mechanical heart supports, where we describe more the classification of mechanical heart supports, their tactics and goals of use, indications and contraindications for the implantation of the support, and we also focus on the surgical aspects of the implantation of this specific support. In the third chapter, the specifics of nursing care for patients with HeartMate 3 are described, both in the preoperative, intraoperative and postoperative periods. The practical starting points of our thesis describe the nursing process of its phases and the nursing process of a patient who was indicated for the implantation of a HeartMate 3 mechanical heart support. Marjora Gordon's model of 13 functional patterns of health was used to process the data of the practical part. At the end of the diploma thesis, we proposed recommendations for practice, which are intended for medical personnel, for the patient himself, but also for his family.
