Problems of a dignified death in a hospital

Abstract:

Cíl: Cílem práce bylo zjistit, jaká je kvalita péče o umírající pacienty v nemocnici z pohledu všeobecných sester a jaká je úroveň uspokojování bio-psycho-sociálních a spirituálních potřeb terminálně nemocných na vybraných odděleních. Metodika: Výzkumný soubor tvořilo 58 respondentů, kteří pracují v nemocnici ve Frýdku-Místku na oddělení ARO, chirurgickém oddělení a interním oddělení. K hodnocení byl vytvořen dotazník s cílenými otázkami k dané problematice. Realizace dotazníkového šetření probíhala v období listopad 2012 - leden 2013. Výsledky: Šetřením bylo zjištěno, že názor respondentů na daných odděleních na kvalitu péče o umírající pacienty je poměrně shodný. Z pohledu důstojnosti posuzují postoj pečujících vůči terminálně nemocným kladně, mají snahu projevovat jim úctu i respekt. Dotazovaní uvedli, že berou ohled na veškeré potřeby umírajících. Většina klade důraz na uspokojování biologických potřeb, na nejvyšší příčku řadí potřebu být bez bolesti. Pouze na oddělení ARO je větší procento respondentů, kteří s umírajícími hovoří zřídka. Přes pozitivní hodnocení péče v nemocnici však respondenti upřednostňují umírání v domácím a klidném prostředí. Závěr: Kvalita péče o umírající pacienty v nemocnici je podle vyjádření respondentů na dobré úrovni, péči hodnotí pozitivně. Respondenti na vybraných odděleních se zamýšlejí nad potřebami umírajících, jejich prožíváním a mají snahu uspokojovat veškeré jejich potřeby i přes vysokou náročnost této péče.

Abstract:

Objective: the aim of this work was to find out what is the quality of care of dying patients in the hospital from the perspective of general nurses and what is the level of satisfaction of the bio-psycho-social and spiritual needs of terminally ill in selected departments. Methodology: a research file consisting of 58 respondents, who work at anaesthesiological-resuscitation department (ARO), surgical department and the internal department of Frydek-Mistek hospital. In order to evaluate the quality of care, a questionnaire was created with targeted questions on the subject. The survey of the questionnaire was conducted during the period from November 2012 to January 2013. Results: the results of the investigation showed that the respondents ' opinion about the quality of care of dying patients is quite similar at all the investigated wards. From the perspective of dignity, the nurses positively consider the attitude of caring in relation to terminally ill patients, with efforts to show them reverence and respect. Questioned nurses said they take all the needs of the dying into their consideration. Most of them put the emphasis on satisfying the biological needs, with the top rank shown to be the need to be pain free. Only at the department of ARO is there a higher percentage of respondents who rarely speak with the dying. Despite of the positive assessment of the care in the hospital, however, respondents prefer patients dying at home and in a peaceful environment. Conclusion: the quality of care of dying patients in the hospital is according to the statements of the respondents at a good level, they evaluate the care positively. Respondents at selected departments think about the needs of the dying, about their experience and by making efforts to satisfy all their needs despite the high level of difficulty of this work.
