The needs of people with the dual diagnosis in therapeutic community

Abstract:

Diplomová práce se zabývá problematikou lidí s duálními diagnózami v terapeutické komunitě. Tato specifická cílová skupina se obtížně dostává do sociálních služeb, jako je např. terapeutická komunita nebo z těchto služeb předčasně vypadává. Autor ve své práci uvažuje o tom, jaké jsou potřeby lidí s duálními diagnózami v terapeutické komunitě a pokládá výzkumnou otázku: „jak interpretují své potřeby lidé s duálními diagnózami v terapeutické komunitě?“. Tuto otázku se práce pokouší objasnit v kvalitativním výzkumu pomocí interpretativní fenomenologické analýzy (IPA) polostrukturovaných rozhovorů s lidmi s duálními diagnózami. Analýza čtyř rozhovorů nachází společná témata napříč rozhovory a ukazuje, jak interpretují potřeby klienti terapeutických komunit. Společnými tématy v rozhovorech jsou téma bezpečí, zájmu a sdílení, sebepoznání a porozumění duševním potížím a také seberealizace. Autor v závěru opatrně vydává doporučení pro pracovníky terapeutických komunit – zaměřit se na zpracování traumat a sdílení nepříjemných emocí, pracovat s medikací jako s jedním z nástrojů – ne nezbytností, otevřeně reagovat na klienta a umožnit mu se svobodně rozhodovat. Abstract:

The thesis deals with the issue of people with dual diagnosis i therapeutic community. This specific target group is difficult to accept in social services like therapeutic community and has also many drop outs from services. The autor considers what are the needs of people with dual diagnosis in therapeutic community and ask the research question: „How people with dual diagnosis interpret their needs in therapeutic community?“ The thesis is trying to answer with qualitative research and Interpretative phenomenological analysis (IPA) of semistructured interviews with people with dual diagnosis. The analysis of four interviews finds out the common themes across the interviews and shows haw clients of therapeutic community interpret their needs. The common themes are: safety, sharing of emocions and concern, self-knowlidge and understanding mental problems and self-realization also. The autor cautiously makes recommendations for social workers in therapeutic communities: focus on treating trauma and sharing of unpleasant emocions, managing madication as one of the tools – not as necessity, openly respond the client and give him the freedom to make decisions.
