Intraarterial Thrombolytic Therapy Complications of Peripheral Arterial Disease in Elderly Patiens

Abstract:

Cíl: Cílem diplomové práce bylo pomocí kvantitativního výzkumu zmapovat výskyt komplikací, které byly zaznamenány u intraarteriální trombolýzy. Tato metoda je jednou z léčebných možností ischemické choroby dolních končetin. Dílčími cíly bylo porovnat výskyt komplikací u seniorů s věkovou skupinou starší dospělosti, zjistit rozdíly ve výskytu komplikací z místa vpichu během aplikace trombolytika a rozdíly ve výskytu komplikací z místa vpichu po vytažení zavaděče u obou věkových kategorií. Dalšími cíli bylo verifikovat, zda na výskyt komplikací má vliv pohlaví, hladina BMI, přístup do tepenného řečiště, lokalizace místa vpichu, doba trvání aplikace trombolytika, pokles hladiny fibrinogenu, pokles hladiny hemoglobinu a druh zvolené komprese místa vpichu. Metodika: K získání dat výskytu komplikací při intraarteriální trombolýze byl vyhotoven hodnotící protokol, do kterého byl denně prováděn záznam jednotlivých položek, které mapovaly oblasti sledovaného problému. Průzkumné šetření probíhalo od 1. 1. 2011 do 31. 12. 2011. Do výzkumného souboru bylo zahrnuto 142 respondentů, kteří byli ve sledovaném období léčeni intraarteriální trombolýzou na angiologické JIP Centra vaskulárních intervencí Vítkovické nemocnice. Pro ověření vztahu výskytu komplikací v závislosti na sledovaných parametrech byl použit ?2 test, Fisherův exaktní test, t-test pro dva výběry. Výsledky: Z průzkumného šetření vyplynulo, s kterými komplikacemi se během lokální intraarteriální trombolýzy setkáváme. V našem výzkumném celku jsme zjistili významné statistické rozdíly v počtu mužů a žen a v jejich hladině BMI. Verifikovali jsme, že věk není podmiňujícím faktorem vzniku komplikací. Dále jsme zjistili souvislost mezi komplikací krvácení z místa vpichu a kontralaterálními přístupy do tepenného řečiště. Prokázali jsme i souvislost mezi dobou aplikace trombolytika a poklesem hladiny fibrinogenu. U komplikací z místa vpichu po vytažení zavaděče jsme prokázali souvislost komplikace krvácení s poklesem hladiny fibrinogenu a ošetření místa vpichu angiosealem. Zjistili jsme, že výskyt rozsáhlých hematomů a rezistence po vytažení zavaděče má souvislost s lokalizací místa vpichu. Závěr: Ošetřování pacientů s cévním onemocněním je vysoce specializovaná péče. Orientace sestry v dané problematice, spolupráce s ostatními členy týmu, participace na tvorbě standardizovaných vnitřních předpisů a jejich dodržování, eliminuje výskyt komplikací, neprodlužuje hospitalizaci a podtrhuje efekt úspěšného endovaskulárního intervenční výkonu.

Abstract:

Aim: The objective of this thesis was to map the incidence of complications reported in connection to intra-arterial thrombolysis with the use of quantitative research. This method is one of the medical procedures used for ischemic disease of lower limbs treatment. Partial objectives were: comparing the incidence of complications in the elderly age group with older adults, learning differences in the incidence of complications from the injection site during application of thrombolytics and differences in the incidence of complications from the injection site after removing the loader at both age categories. Other objectives were to verify whether the occurrence of complications is influenced by gender, BMI levels, access to the arterial bed, injection site localization, duration of thrombolytics application, decreasing of fibrinogen level, decreasing of hemoglobin level and the type of compression at the injection site. Methodology: To obtain the data on the incidence of complications during intraarterial thrombolysis, evaluation protocol was drawn up. Each item, which explored the problem of reference, was recorded there on daily basis. Exploratory investigation period ran from January 1, 2011 to December 31, 2011. The research sample involved 142 respondents who were cured by intra-arterial thrombolysis at the Angiology ICU in the Center for vascular interventions of Vítkovice Hospital. To verify the relationship between complications occurence and the monitored parameters ?2 test, Fisher's exact test and t-test for two samples were used. Results: The exploratory investigation showed complications which may occur during local intra-arterial thrombolysis. In our research group we found significant statistical differences in the number of men and women and their BMI levels. We verified the fact that age is not the determining factor for complications. We also found association between bleeding from the injection site and contralateral acces into the arterial circulation. We proved the connection between thrombolytic application time and decrease of fibrinogen levels. We also established coherrence between the complication of bleeding with the decrease in fibrinogen level and angiosealem treatment at complications from the injection site after removing the loader. We found that the incidence of large hematomas and resistance after removing loader is linked to the injection site localization. Conclusion: Treatment of patients with vascular disease is a highly specialized care. The comprehence of nurses at the issue, cooperation with other team members and participation in creating standardized internal regulations and their compliance eliminates complications, reduces hospitalization and emphasizes the effect of successful endovascular intervention performance.
