Renal cell carcinoma: Biology, treatment and protein biomarkers

Abstract:

Renální karcinom (RCC) je zhoubné onemocnění ledvin. Většina nádorů je histologicky řazena do světlobuněčného subtypu následovaného subtypy papilárním a chromofobním. RCC pacienti s lokalizovaným onemocněním jsou léčeni nefrektomií. U menší části z nich dochází ke generalizaci onemocnění ve formě metastatického RCC (mRCC), u níž je využíváno cílených biologických přístupů především na bázi tyrosinkinázových a mTOR inhibitorů a imunoterapie. V dosavadních studiích byla identifikována řada proteinových biomarkerů s diagnostickým a terapeutickým potenciálem u RCC, jejichž přehled poskytuje rešeršní část této práce. V části experimentální je provedena analýza hladin 38 těchto proteinů na souborech 86 vzorků lokalizovaných a 75 metastatických RCC tumorů ve vztahu ke klinickopatologickým charakteristikám. U vzorků lokalizovaných RCC nádorů byla zjištěna statisticky významná změna hladin biomarkerů u 11 proteinů ve vztahu ke grade dle Fuhrmanové, u 10 proteinů ve vztahu k velikosti nádoru, u 4 proteinů ve vztahu k performance status, u 7 proteinů ve vztahu k relapsu a u 2 proteinů ve vztahu k míře relapsu. U vzorků metastatických RCC nádorů byla statisticky významná změna hladin biomarkerů zjištěna u 3 proteinů ve vztahu k velikosti nádoru, u 13 proteinů ve vztahu k histologickému subtypu RCC, u 20 proteinů ve vztahu k nádorové tkáni, u 10 proteinů ve vztahu k odpovědi na léčbu a u 3 proteinů ve vztahu k MSKCC score. Lze shrnout, že proteinové biomarkery popsané literární rešerší a analyzované v experimentální části jsou významně spojeny s klinickopatologickými charakteristikami RCC.

Abstract:

Renal cell carcinoma (RCC) is a malignant kidney disease. Most tumors are histologically classified into the clear cell subtype followed by the papillary and chromophobic subtypes. RCC patients diagnosed with localized disease are treated with nephrectomy. In a smaller part of them, the disease is generalized in the form of metastatic RCC (mRCC), in which targeted biological approaches are used, mainly based on tyrosine kinase and mTOR inhibitors and immunotherapy. In previous studies, number of protein biomarkers with diagnostic and therapeutic potential in RCC have been identified. The literature review provides us an overview of these biomarkers. In the experimental part, the analysis of the levels of 38 protein biomarkers was performed in the sets of 86 samples of localized and 75 ones of metastatic RCC tumors in relation to clinicopathological characteristics. In localized RCC, statistically significant changes in biomarker levels were found for 11 proteins in relation to Fuhrman grade, in 10 proteins in relation to tumor size, in 4 proteins in relation to performance status, in 7 proteins in relation to relapse and in 2 proteins in relation to relapse rate. In mRCC, a statistically significant change in biomarker levels was found in 3 proteins in relation to tumor size, in 13 proteins in relation to the histological subtype of RCC, in 20 proteins in relation to tumor tissue, in 10 proteins in relation to treatment response and in 3 proteins in relation to the MSKCC score. The protein biomarkers described in literature review and analyzed in the experimental part are significantly associated with clinicopathological parameters of RCC.
