A mother expectants view on spinal versus general anaesthetics et delivery finished by caesaren section

Abstract:

Cíl: Cílem práce bylo zjistit pohled rodiček na celkovou nebo spinální anestézii u porodu ukončeného císařským řezem. Metodika: Průzkumný soubor tvořila 120 respondentek, které porodily císařským řezem v celkové a spinální anestézii. Průzkumné šetření bylo prováděno od října 2009 do únoru 2010, bylo realizováno v Městské nemocnici Ostravě, metodou dotazníku. Výsledky: Šetřením bylo zjištěno, že informace o spinální anestézii nezískávají ženy od svého obvodního gynekologa, ale v příjmové ambulanci nemocnice, kde ženy rodí. V případě dalšího porodu ukončeného císařským řezem ve spinální anestézii by si ženy opět vybraly spinální anestézii. Taky byly uvedeny nejčastější pocity při spinální anestézii tahy a tlaky. Ale ženy, které rodily císařským řezem v celkové anestézii by si celkovou anestézii nezvolily, neboť to ve většině případů bylo v akutní situaci, při ohrožení života dítěte či matky. Posledním kladným zjištěním bylo doporučení porodu císařským řezem v celkové anestézii známým. Závěr: Průzkumným šetřením bylo zjištěno, že ženy si plánují a jsou informovány o způsobu ukončení porodu, ale i o typu anestézii. Mají zájem o zážitek na tento den příchodu potomka do rodiny. Ženy se rozhodují na základě informací od personálu a celkovou anestézii by již v případě dalšího porodu nevyužily, ale ženy, které prodělaly porod ve spinální anestézii by tuto možnost znovu využily. Zde je nutno podotknout, že při akutním císařském řezu, ženám nebyla spinální anestézie nabídnuta! Ze zkušeností z praxe se potvrdilo, že nejčastější pocity při spinální anestézii jsou tahy a tlaky. Ale v rámci doporučení svým známým by celkovou i spinální anestézii u císařského řezu doporučily obě sledované skupiny a to i na základě spokojenosti s chováním zdravotnického personálu, což je důležitá pozitivní zpětná vazba pro všechen zdravotnický personál.

Abstract:

Objective: The objective of this thesis was to find out the opinions of mothers about general and spinal anesthesia for giving birth by caesarian section. Methodology: The research respondent group was made of 120 mothers who gave birth to their babies by caesarian section in both general and spinal anesthesia. The research enquiry was conducted from October 2009 till February 2010 in Municipal Hospital in Ostrava by a questionnaire method. Outcomes: The enquiry found out that women did not get information about spinal anesthesia from their general gynecologists but in the hospital admission office where they gave birth. In case of further deliveries with a caesarian they would choose spinal anesthesia again. The most common sensations felt in spinal anesthesia were pulls and pressures. However, women who gave birth to their babies by the caesarian section in general anesthesia would not chose the general anesthesia again because in most cases it was an acute life-threatening situation for the mother or the child. The last positive finding was recommendation of general anesthesia to relatives and friends. Conclusion: The research enquiries found out that women plan and are informed about both the delivery and also the type of anesthesia. They are interested in the experience of their baby coming to the world. Their decisions are based on the information gained from the nursing staff and they would not choose a general anesthesia in case of further delivery, however, women who underwent a spinal anesthesia would choose it again. It needs to be mentioned that in case of an acute cesarean section, the women were not offered a spinal anesthesia! Experience confirmed that the most common sensations felt in spinal anesthesia were pulls and pressures. However, in terms of recommendation to their relatives and friends, both respondent groups would recommend both spinal and general anesthesia based on their satisfaction with the nursing staff behavior, which is a positive feedback for all healthcare personnel.
