Palliative care in the conditions of residential social service for the elderly people.

Abstract:

Diplomová práce se zabývá tématem paliativní péče v podmínkách pobytové sociální služby pro seniory. Cílem je analyzovat motivaci a bariéry v implementaci paliativní péče do praxe pobytových zařízení pro seniory v Moravskoslezském kraji. Práce je členěna do šesti kapitol. První kapitola pojednává o thanatologii, umírání a institutu Dříve vysloveného přání. Druhá kapitola se zabývá paliativní péčí v kontextu praxe v zahraničí i v České republice, popisuje legislativní ukotvení, rozvoj a dostupnost, zásady a etiku při poskytování paliativní péče. Třetí kapitola představuje paliativní péči v podmínkách pobytové sociální služby pro seniory. Čtvrtá kapitola se věnuje kvalitě života s ohledem na specifika života seniorů. Pátá kapitola se zabývá metodikou výzkumu včetně vymezení cílů a hypotéz výzkumného šetření. Poslední kapitola obsahuje výsledky a interpretaci dat. Výsledky jsou vzhledem k velikosti výzkumného souboru pouze indikativní. Výsledky výzkumného šetření ukazují, že koncept paliativní péče je zaveden u většiny respondentů, není však nalezen rozdíl mezi typem zařízení ani mezi typem zřizovatele. Z výsledků lze dále pozorovat, že zavedení paliativní péče do praxe nemá vliv na snížení procenta úmrtnosti klientů v hospitalizacích. V oblasti motivace k implementaci paliativní péče lze pozorovat motivační faktory interní a externí. Mezi interní impulsy jsou řazeny vlastní zkušenosti s potřebou paliativní péče či vnitřní nastavení skrze křesťanské hodnoty. Mezi externími impulsy respondenti uvádí podněty ze strany klientů sociálních služeb a vedení organizace či zřizovatele. Výsledky dále ukazují na možné bariéry v zavádění či poskytování paliativní péče. Mezi hlavní bariéry patří zejména nedostatek kvalifikovaného personálu, chybějící návazné služby a nedostatečné finanční zdroje.

Abstract:

The diploma thesis looks into the topic of palliative care in the conditions of residential social services for the elderly people. The main aim is to analyze the motivation in the implementation of palliative care in the practice of residential facilities for the elderly people in the Moravian-Silesian region. The thesis is divided into six chapters. The first chapter deals with the thanatology, dying and the institute of the previously expressed wish. The second chapter deals with palliative care in the context of practice abroad and in the Czech Republic, describes the legislative anchoring, development and availability, principles and ethics in the provision of palliative care. The third chapter presents palliative care in the conditions of residential social services. The fourth chapter deals with the quality of life with regard to the specifics of the lives of elderly people. The fifth chapter deals with the research methodology, including the definition of objectives and hypotheses of the research survey. The last chapter contains the results and interpretation of the data. The results are only indicative due to the size of the research group. The results of the research investigation show that the palliative care concept had been implemented for most of the respondents, however there was not found any difference between the type of the institution and the type of the establisher. The results also show that the introduction of palliative care in practice does not affect the reduction of the mortality rate of clients in hospitalizations. Concerning motivation, the implementation of palliative care, internal and external motivational factors can be observed. The internal impulses include one's own experience with the need for palliative care or internal setting through Christian values. Among the external impulses, mentioned by respondent's suggestions from clients of social services and the management of the organization or the founder. The results also point to possible barriers in the introduction or provision of palliative care. The main barriers include, in particular, a lack of qualified staff, a lack of follow-up services and insufficient financial resources.
