The Benefit of Tower of London (Drexel version) in the Diagnostics of Executive Functions in Schizophrenia

Abstract:

Hlavním cílem této výzkumné studie bylo ověření přínosu testu Londýnské věže (ToL) k diagnostice dysexekutivního deficitu u schizofrenie. Rozlišovací schopnost testu byla ověřována na souboru 19 kognitivně a neurologicky zdravých jedinců (NK) a na souboru 19 osob se schizofrenií (Sch). Osoby byly rozděleny do párů 1:1 na základě shodných nebo podobných demografických charakteristik, aby byl vliv intervenujících proměnných snížen na minimum. Zkoumali jsme také rozdíly v rozlišovací schopnosti dvou různých variant ToL, konkrétně původní Shallicovu variantu (ToL-SH) a přepracovanou Drexel verzi (ToLDX). Pro signifikantní rozdíly mezi skupinami je provedena ROC (Reciever Operating Characteristic) analýza ve snaze stanovit optimální screeningové cut-off skóry. Diskriminační schopnost ToLDX k odlišení dysexekutivního deficitu od jeho absence vyšla na spodní hranici statistické významnosti. Dva skórovací systémy ToL-SH (Shallicův druhý skórovací systém a alternativní skórovací systém dle Andersona) ve výsledku signifikantně rozlišovaly mezi skupinami NK a Sch a stanovili jsme pro ně cut-off skóry. Výsledky této studie tedy naznačují, že obě varianty, zvláště pak variantu ToL-SH, lze považovat za validní nástroj k diagnostice exekutivního deficitu, ale zároveň je vhodné, aby byly součástí komplexní neuropsychologické testové baterie.

Abstract:

The main purpose of this research was to verify the benefits of the London Tower test (ToL) in the assessment of dysexecutive deficit in schizophrenia. Discriminant validity of the test was investigated in a sample of 19 cognitively and neurologically healthy individuals (NK) and a sample of 19 patients with schizophrenia (Sch). Subjects were divided into pairs 1:1 on the basis of identical or similar demographic characteristics that the influence of intervening variables was minimized. We also researched whether the two Tower of London tasks – Shallice’s ToL (ToL-SH) and ToL-Drexel (ToL-DX) have equivalent discriminant validity in differentiating between research groups. In order to determine screening cut-off scores, ROC (Receiver Operating Characteristic) analysis is used. Performance of Sch group in ToLDX task differs from the NK population on the lowest level of significance. Our analysis showed that two scoring systems on Tol-SH task (Shallice’s second scoring system and alternative scoring system according to Anderson) significantly differentiate between NK and Sch, so we determined optimal cut-off scores. The results of the present study indicate that both variants, especially variant Tol-SH, can be considered as a valid tool for assessing dysexecutive deficit, but it is also recommended that they are a part of a complex neuropsychological battery.
