Nursing problematics patient with a peripheral nerve block

Abstract:

Kromě běžných postupů léčby pooperační bolesti podáváním analgetik či opioidů, máme k dispozici svodnou anestezii. Periferní nervová blokáda je metoda, která je dnes velmi často oblíbená a vyhledávaná. Využíváme ji k operacím na horních nebo dolních končetinách, kde není potřeba dosáhnout k znecitlivění celé končetiny. Techniku je možné použít i na trupu a břiše, například na operaci prsu. Tato metoda spočívá v aplikaci injekce lokálního anestetika do blízkosti nervu či nervových pletení. To nám zajistí značný vliv na pooperační bolesti. Teoretická část mé bakalářské práce začíná stručnou historií a anatomií. Poté popisuji periferní nervové blokády. Výhody, nevýhody, indikace, kontraindikace a techniky kterými lze nervové struktury vyhledat. Poté popisuji přípravu pacienta podstupující typ anestezie a ošetřovatelkou část zdravotní sestry. Péči o pacienta před provedením blokády, během ní a i poté. V práci se lehce zaměřuji i na lokální anestetika. V poslední teoretické části nastiňuji typy blokád, které nejčastěji využíváme na horních a dolních končetinách. Praktická část je zaměřena na průzkum pomocí dotazníků, které byly distribuovány po svolení vedoucího pracovníka traumatologického a anesteziologického oddělení pacientům, kteří periferní nervovou blokádu podstoupili. Cílem průzkumu bylo zjištění, jak jsou pacienti spokojeni s PNB. Zda byl pacient dostatečně informován o průběhu, výhodách či možných komplikacích a jestli vše zmíněné odpovídalo skutečnému průběhu. Jaké pocity měl pacient v operované končetině v souvislosti s periferní nervovou blokádou a následná intenzita pooperační bolesti. V poslední části průzkumu se zaměřuji na pacientův pooperační komfort a jestli by s touto technikou souhlasil v případě dalšího operačního zákroku či nikoli.

Abstract:

In addition to the usual procedures for treating postoperative pain by administering analgesics or opioids, we have anaesthesia available. Peripheral nerve blockade is a method that is very popular and sought after today. We use it for operations on the upper or lower limbs where it is not necessary to reach the entire limb to numb it. The technique can also be used on the trunk and abdomen, for example for breast surgery. This method involves injecting a local anaesthetic near the nerve or nerve plexus. This will provide a significant effect on postoperative pain. The theoretical part of my bachelor thesis starts with a brief history and anatomy. I then describe peripheral nerve blockades. Advantages, disadvantages, indications, contraindications and techniques by which nerve structures can be located. I then describe the preparation of the patient undergoing the type of anesthesia and the nursing portion of the nurse. The care of the patient before, during and after the blockade. I also focus lightly on local anaesthetics. In the last theoretical section I outline the types of blockades that are most commonly used on the upper and lower extremities. The practical part focuses on a survey using questionnaires that were distributed, with the permission of the head of the trauma and anaesthesia department, to patients who underwent peripheral nerve blockade. The aim of the survey was to find out how satisfied patients are with PNB. Whether the patient was sufficiently informed about the procedure, the benefits or possible complications and whether everything mentioned corresponded to the actual procedure. What sensations the patient had in the operated limb in relation to the peripheral nerve block and the subsequent intensity of postoperative pain. In the last part of the survey I focus on the patient's postoperative comfort and whether or not he would agree to this technique in case of further surgery.
