Relationship between stress and cognitive functions in early phases of schizophrenia.

Abstract:

Součástí schizofrenie je postižení kognitivních funkcí a dle některých studií i vyšší hladiny kortizolu. Ve srovnání s lidmi po prodělaných několika epizodách schizofrenie (CHS) je u prvních epizod schizofrenie (FES) menší postižení pracovní paměti, verbálního a vizuálního učení a sociální kognice ale větší postižení pozornosti/bdělosti. Některé studie však nepotvrzují větší postižení pozornosti/bdělosti a spíše poukazují na pracovní paměť jako na více postiženou doménu. Zároveň je dávána do souvislostí hladina kortizolu a pracovní a verbální paměť u FES. Se symptomatickým zlepšením dochází dle některých autorů ke snížení hladin kortizolu. Dalším hormonem, jehož hladina může odrážet psychopatologický obraz, je aldosteron. Prvotní studie ukazují, že hladina aldosteronu vzrůstá se zlepšením symptomů. Cílem této práce bylo popsat charakter kognitivního deficitu a klinického obrazu a hladin stresových hormonů u FES. Bylo zahrnuto sedm pacientů. Nízký počet respondentů neumožňoval použití statistických metod, z toho důvodu byly stanoveny hypotézy z literatury. Šest pacientů mělo diagnózu akutní polymorfní psychotické poruchy se symptomy schizofrenie a jeden s paranoidní schizofrenií, v remisi. Ve výzkumném vzorku bylo šest žen a jeden muž o průměrném věku 23,57 let (směrodatná odchylka 4,27). Všichni pacienti byli medikováni. Zahrnuti byli pacienti, kteří vyhledali pomoc na Psychiatrické klinice Fakultní nemocnice Brno. Pro posouzení kognitivního profilu byla použita baterie Matrics Consensus Cognitive Battery (MCCB) a klinický obraz byl posouzen pomocí Clinical Global Impression Severity (CGI-S). Hladiny stresových hormonů odebíral nemocniční personál z krve. Aldosteron byl odebírán ráno v šest hodin, kortizol pak ve čtyři hodiny odpoledne. Hladiny hormonů byly rovněž posuzovány po ročním vyšetření. Pro všechny pacienty byla vypracována kazuistika za účelem lepšího pochopení možných intervenujících proměnných. Profil deficitu byl vypočítán pomocí programu MCCB a skóry byly shrnuty v tabulkách a obrázcích. Jedna pacientka odmítla kontrolní vyšetření po roce. Snížení hladin kortizolu spolu se zlepšením symptomů bylo pozorováno u třech pacientů, pouze u jedné pacientky došlo zároveň ke zvýšení hladin aldosteronu. U třech pacientů pak došlo ke zvýšení obou hormonů. Z výsledků kognitivního vyšetření bylo pouze u jedné pacientky nejvýraznější postižení pracovní paměti a rychlosti zpracování informací. U pěti pacientů byla nejvíce postižena pozornost/bdělost a v této doméně byl nalezen nejnižší t-skór napříč výzkumným vzorkem. Rozporuplné výsledky byly pozorovány u souvislostí hladin kortizolu a pracovní a verbální paměti. Pro pracovní paměť nebylo nalezeno žádné zastoupení. U verbální paměti měla jedna pacientka spolu s nejvýraznějším postižením i zvýšenou hladinu kortizolu, ačkoli pouze o jeden t-skór níže skóroval pacient s o poznání nižší hladinou kortizolu. Vztah kortizolu a symptomatologického obrazu je napříč vzorkem rozporuplný. U aldosteronu došlo u třech pacientů ke zvýšení hladin, ale zároveň se u nich zvýšil i kortizol. Nejvíce postiženou doménou byla pozornost/bdělost, pouze u jedné pacientky to byla pak pracovní paměť a rychlost zpracování informací. Vztah verbální paměti a kortizolu byl pozorován pouze u jedné pacientky. Asociace pro pracovní paměť pozorována nebyla.

Abstract:

Cognitive impairments and lower levels of cortisol are an integral part of schizophrenia. In comparison people with multiple episode schizophrenia (CHS), the first episode schizophrenia patients (FES) present less pronounced impairment in working memory, verbal and visual learning and social cognition but more severe impairment in attention/vigilance. However, several studies have found attention/vigilance to be less impaired and more severe deficit in working memory to be present. At the same time there seems to be a relationship between cortisol values and working memory and verbal memory in FES. Some authors suggests that with symptomatic improvement goes hand in hand reduction in cortisol levels. Aldosterone and its implications for symptom improvement has also been considered. Pilot studies are showing that values of this hormone increases with symptomatic improvement. The aim of this study was to characterise the profile of cognitive deficits and clinical picture with stress hormone values in FES. Seven patients were included. These numbers did not justify the usage of statistical analysis. Hypotheses were postulated based on the literature. Six patients received diagnosis of acute polymorph psychotic disorder with symptoms of schizophrenia and one with paranoid schizophrenia, currently in remission. Within the research sample there were six women and a man, mean age of 23,57 years (standard deviation = 4,27). All patients were medicated. Included were patients who seeked help at Psychiatric Clinic of the University hospital in Brno. Cognitive profile was evaluated using Matrics Consensus Cognitive Battery (MCCB) and clinical picture with Clinical Global Impression Severity (CGI-S). Levels of stress hormones were assessed by trained staff at the hospital. Aldosterone was taken at six A.M. and cortisol at four P.M. Stress hormones were also evaluated one year from the admission. A case study was written for each patient to better understand possible confounding variables. Profile of the deficit was calculated based on MCCB programme and scores were summarised in tables and pictures. One patient denied control evaluation after one year, so stress hormones were not assessed for her. Lower cortisol values were observed together with symptomatic improvement in three patients and only one patient showed an increase of aldosterone at the same time. Three patients showed an increase of both hormones. Results of cognitive evaluation show most profound deficit of working memory and speed of information processing in one patient. Attention/vigilance was most severely impaired in five patients with the lowest t-score across the research group. Conflicting results were found for cortisol values and working memory and verbal memory. No representation was found for working memory. The most severe verbal memory impairment together with higher cortisol value was found in one patient, although a patient with significantly lower cortisol value scored only one t-score lower. The relationship between cortisol and symptoms of schizophrenia is contradictory. For aldosterone, three patients showed higher values with symptomatic improvement together with higher values of cortisol. The most impaired domain of cognition was attention/vigilance. Only for one patient it was working memory and speed of information processing. Relationship between verbal memory and cortisol was observed in one patient. No association was found for working memory and cortisol.
