Tracheostomy care in intensive care from a nurse's perspective

Abstract:

Úvod: Bakalářská práce se věnuje péči o tracheostomii v intenzivní péči z pohledu sestry. Zaměřuje se na zásady péče o tracheostomii na jednotkách intenzivní péče a jejich dodržování. Tato práce je rozdělena na teoretickou a praktickou část. V teoretické části je popsána anatomie a fyziologie dýchacích cest, tracheostomie a pojmy s ní spojené, v neposlední řadě popisuje ošetřovatelskou péči o tracheostomii. Pro praktickou část byla použita metoda dotazníkového šetření. Cíl práce: Cílem bakalářské práce bylo zjistit znalosti zdravotních sester v péči o tracheostomii na odděleních intenzivní péče. Dílčí úkoly zněly: Zjistit, jaký typ odsávání je nejčastěji používán; Ohodnocení znalostí sester v odsávání z dýchacích cest. V práci byly stanoveny 2 hypotézy. Metodika: Praktická část byla zpracována formou kvantitativního výzkumu nestandardizovaným dotazníkem. Dotazník byl rozeslán online do 3 krajských nemocnic. Zpracování dat proběhlo pomocí Microsoft Excel, tato data byla pro lepší přehled uspořádána do tabulek a grafů. Hypotézy byly statisticky ověřeny pomocí Studentova t - testu. Výsledky: Z výsledků vyplývá místy značná absence znalostí sester v péči o tracheostomii. Respondenti byli dle bodového zisku ohodnoceni známkou od 1-5, kdy 1 je nejlepší. Tohoto ohodnocení dosáhlo pouze 7 respondentů, nejvíce respondentů získalo ohodnocení 2 a 3. Pro toto zjištění bylo vytvořeno doporučení pro praxi s názvem Desatero pravidel v péči o tracheostomii. V případě obou hypotéz jsme nezamítli nulovou hypotézu. Závěr: Znalosti sester jsou v určitých oblastech nedostatečné, i přes to že péče o tracheostomii patří mezi základní kompetence sester na odděleních intenzivní péče.

Abstract:

Introduction: The bachelor thesis focuses on tracheostomy care in intensive care from the nurse's perspective. It focuses on the principles of tracheostomy care in intensive care units and their compliance. This thesis is divided into theoretical and practical parts. The theoretical part describes the anatomy and physiology of the airway, tracheostomy and the concepts associated with it, and last but not least it describes the nursing care of tracheostomy. The practical part used a questionnaire survey method. Aim of the thesis: The aim of the bachelor thesis was to determine the knowledge of nurses in tracheostomy care in intensive care units. The sub-tasks were: To find out which type of suction is most commonly used. To assess nurses' knowledge in airway suctioning. In this study, 2 hypotheses were set. Methodology: the practical part was done in the form of quantitative research, using a non-standardized questionnaire. The questionnaire was distributed online to 3 regional hospitals. Data processing was done using Microsoft Excel, the data were organized in tables and graphs for better overview. Hypotheses were statistically tested using Student's t - test. Results: The results show a significant lack of knowledge in tracheostomy care. Respondents were scored on a scale of 1-5, with 1 being the best. Only 7 respondents achieved this rating, with most respondents scoring 2 and 3. A practice guideline called Ten Rules in Tracheostomy Care was developed to address this finding. Conclusion: Nurses' knowledge is inadequate in certain areas, despite the fact that tracheostomy care is one of the core competencies of nurses in intensive care units.
